About Niagara Falls Community Health Centre

At Niagara Falls Community Health Centre (NFCHC), we are more than a healthcare facility. We are a community hub dedicated to improving the lives of people who face barriers to care.

Our diverse and inclusive team provides holistic, community-based healthcare with a focus on health equity, prevention, and addressing the underlying factors that influence health and well-being. We are committed to creating a supportive work environment that values collaboration, professional growth, work-life balance, and employee well-being.

Position Overview

The Social Worker, 2S&LGBTQ+ Health and Gender-Affirming Care, plays an important role in improving access to inclusive, affirming, and comprehensive healthcare for Two-Spirit, lesbian, gay, bisexual, transgender, queer, and other sexual- and gender-diverse (2S&LGBTQ+) communities across Niagara.

Working as part of an interdisciplinary team, this position combines clinical social work services with community development, health promotion, group programming, and systems-level advocacy. The successful candidate will provide individualized psychosocial support, counselling, crisis intervention, and care navigation while helping clients access gender-affirming healthcare and other community resources.

Beyond direct client care, the Social Worker will contribute to program development, community engagement, partnership building, and organizational initiatives that address barriers to care and improve health outcomes for 2S&LGBTQ+ communities. This role is well suited to a clinician who values both meaningful one-on-one client relationships and the opportunity to create broader, lasting change within the community.
