Key Responsibilities
● Manage and deliver patient care plans collaboratively within a team
● Provide wound care services to patients in the coverage area
● Maintain accurate and timely charting and documentation
● Use EMR software and mobile devices for patient care management
● Drive to patient locations within the assigned coverage area

Schedule and Shift Details
● Monday – Friday, 8:30 AM – 5:00 PM; no weekends, holidays, or on-call; typically sees 6-7 patients per day

● Must live within 30 minutes of coverage area; approximately 20% driving to patient locations

Practice Partner EMR
Valid Licensed Practical Nurse (LPN) in the state of Illinois (IL)
Valid Certified Wound Ostomy and Continence Nurse (CWOCN)
Valid Regular Driver License (DL) in the state of Illinois (IL)
