PURPOSE OF THE POSITION
The purpose of this position is to implement GreatCare® to ensure the recruitment, retention, and development of the best caregivers and ensure satisfaction of clients through delivery of applicable care solutions and active involvement.
COMPENSATION
$42,000-$48,000 per year
SCOPE
The Care Manager reports to the Agency Director and is responsible for ensuring the satisfaction of both the clients and the caregivers. The incumbent’s responsibilities include a broad range of interaction with caregivers and clients.
PRIMARY DUTIES
- Continually seek out and hire qualified applicants according to office recruitment plan
- Complete the hiring process
- Conduct new hire orientation to caregivers
- Ensure all caregivers complete background checks and registry checks prior to contact with clients
- Conduct employee and client chart audits
- Enroll Caregivers in GreatCare® certification courses
- Create and manage Client schedules
- Match Caregivers with clients
- Review Caregiver visit notes, and clock in and out time
- Manage Caregivers to include: interactions, incidents, disputes, discipline and terminations, per office policy.
- Conduct client care consultations, home safety assessments, and supervisory visits
- Initiate QAPI reporting for infection control, complaints, incidents, and accidents within 24 hours of occurrence
- Conduct Caregiver to client orientations
SHARED DUTIES
- Answer Care Opportunity Calls
- Provide on-call coverage for nights and some weekends
- Perform the duties of a caregiver when necessary
- Maintain client and employee files
- Participate in weekly team meetings
- Conduct caregiver disciplinary actions and terminations
- Perform the duties of a caregiver when necessary

QUALIFICATIONS
• Must be 18 years of age
• Must be able to speak, read, and write the English language
• Must have transportation in good working order
• Must have demeanor and social skills that would enable them to communicate well with clients to achieve favorable outcomes
• At least one-year experience or certified training in care for individuals with functional disabilities
PHYSICAL REQUIREMENTS
• Light (ability to lift 50 lbs)
• Ability to drive a motorized vehicle
• Ability to sit/stand for prolonged periods
WORK HOURS
• 8:00am to 5:00pm Monday through Friday
• Occasional evenings or weekends as required
• On-call duties regularly required
