Lead Physical Therapist

Overview

 

Catholic Health Home Care is seeking a Lead Physical Therapist to provide clinical leadership and high-quality physical therapy services to patients in their homes. The Lead Physical Therapist oversees therapy services, supports and evaluates physical therapy staff, manages an assigned caseload, and ensures compliance with agency policies, professional standards, and federal and New York State regulations.

Responsibilities

Key Responsibilities

• Provide clinical direction, guidance, and support to physical therapists.
• Assess and reevaluate patients and develop individualized plans of care based on physician orders and patient needs.
• Manage patients from admission through discharge and coordinate care with physicians and the interdisciplinary team.
• Develop and instruct patients and caregivers on individualized home exercise programs.
• Monitor patient visits, therapy authorizations, coverage, and quality of care.
• Conduct field evaluations and assist with staff orientation, competencies, education, and preceptorship.
• Review clinical documentation for accuracy, completeness, and timeliness.
• Complete OASIS assessments and required documentation in accordance with agency standards.
• Identify changes in patient condition and needs for additional services.
• Promote patient safety, infection prevention, quality improvement, and service excellence.
• Maintain professional standards, required training, licensure, and certifications.

Qualifications

• Graduate of an approved Physical Therapy curriculum approved by the council of Medical Education of the American Medical Association and the American Physical Therapy Association 
• Minimum 2 years of experience as a licensed Physical Therapist.
• Current New York State Physical Therapy license.
• Computer proficiency.
• Strong clinical, communication, organizational, and leadership skills.
• Ability to work independently and collaboratively with an interdisciplinary team.

Pay Range

USD $110,000.00 - USD $130,000.00 /Yr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
