Description
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one physical therapy treatments.
- Help patients improve strength, balance, mobility, endurance, and functional independence.
- Provide gait training, transfer training, therapeutic exercise, and fall-prevention interventions.
- Recommend assistive devices and adaptive equipment when appropriate.
- Educate patients and caregivers on exercises, mobility strategies, and safety.
- Monitor patient progress and adjust treatment plans as clinically appropriate.
- Complete accurate and timely clinical documentation.
- Communicate with physicians, caregivers, facility staff, and members of the interdisciplinary care team.
- $45–$50 per visit.

- Geriatric or facility-based therapy experience is preferred but not required.
