Outcome Specialist Coordinator

KenCrest is searching for an Outcome Specialist Coordinator to join our Supported Independent Living team!

As an Outcome Specialist Coordinator, you will partner with individuals, their families, and interdisciplinary team members to identify meaningful outcomes, monitor progress toward ISP goals, coordinate required assessments, and ensure documentation accurately reflects the quality of services and supports provided. This position is ideal for someone who is organized, collaborative, detail-oriented, and passionate about helping individuals with intellectual disabilities and autism achieve their goals.

Some Essential Functions of the role:

• Partner with individuals, families, Supports Coordinators, and interdisciplinary team members to coordinate services and promote consistent, person-centered support.
• Create and distribute Outcome Progress Notes in alignment with annual ISP updates and support staff with proper documentation practices.
• Schedule and complete quarterly Universal Outcome Tool reviews and annual assessments in collaboration with individuals and their support teams.
• Review ISP outcomes and action steps, monitor progress toward goals, and provide input on changes that may support positive goal achievement.
• Monitor Outcome Progress Notes, assessments, and related records for accuracy, completeness, consistency, and timely completion.
• Incorporate applicable HRST Service and Training Considerations into Outcome Progress Note documentation.
• Compile and distribute reports and assist with documentation needed for county, state, ODP, and internal audits.
• Collect, review, and report data supporting department KPIs and identify opportunities to strengthen service quality and individual outcomes.
• Maintain compliance with applicable regulatory requirements, HIPAA standards, and KenCrest policies and procedures.
• Participate in team meetings, professional development, Charting the LifeCourse initiatives, and other department or agency initiatives as needed.
• Other duties as assigned.

Minimum Requirements:

• Bachelor’s degree in Human Services or a related field from an accredited college or university, required.
• Two years of experience working directly with individuals with intellectual disabilities and autism preferred.
• Valid driver’s license and reliable transportation.
• Ability to successfully complete required criminal history and child abuse clearances in accordance with applicable state requirements.
• Strong communication, organization, documentation, and collaboration skills.
• Comfort using technology, including a laptop, cell phone, Microsoft Outlook, Word, Excel, Teams, and Evolv.
• Ability to maintain confidentiality and HIPAA compliance across traditional, mobile, and shared work environments.
