JOB SUMMARY:Within the context of a patient, client and family centred model of care and, in accordance with the Mission, Vision and Values, and strategic directions of Providence Health Care, promotes a safe, respectful, and civil working environment for patients, residents, families, visitors and staff. Reporting to the Patient Care Manager, position practices to full scope as per the Standards of Practice outlined by the British Columbia College of Nurses and Midwives (BCCNM), as well as within a client and family-centered care model approach. The Licensed Practical Nurse (LPN) functions as an independent practitioner while recognizing situations, such as increased patient acuity or care, fall outside the scope of an LPN. Accesses the RN for collaborative support and/or clinical intervention as appropriate. Assists patients in meeting their physical, social, spiritual and psychological needs consistent with the mission, vision and values of Providence Health Care (PHC) and established protocols. Working collaboratively with the multidisciplinary team, the LPN performs assessments and plans, implements and provides personal and nursing care to clients, according to organizational policies, procedures, standards and unit specific protocols. The scope or extent of the skills that the LPN performs varies according to the predictability of the outcome for a given client population. Participates in a variety of meetings, including care conferences, and assists with the orientation of new staff and students as required. Responds to and reports incidents according to established procedures. Performs clerical duties as needed.QUALIFICATIONS:Education, Training, and ExperienceGraduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience. Registration with the BC College of Nurses and Midwives (BCCNM) as a practicing LPN registrant. Current basic cardiac life support certification. Advanced preparation in GI Endoscopy that includes completion of recognized post-basic education in gastroenterology/GI endoscopy nursing and a certificate of completion from the Association of Perioperative Registered Nurses (AORN) in gastroenterology, or an equivalent combination of education, training and experience. Preference given to those with recent related GI endoscopy experience. Skills and Abilities- Demonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service.- Ability to work as a member in an interdisciplinary team.- Ability to apply decision-making and analytical skills.- Ability to role model conflict resolution strategies.- Ability to prioritize and organize work.- Ability to teach and identify patient learning needs.- Ability to communicate effectively, both verbally and in writing. - Ability to deal with others effectively. - Ability to operate related equipment. - Physical ability to carry out the duties of the position.1. Provides safe and respectful nursing care to an assigned group of patients, according to established policies and procedures, in combination with evolving individualized care plans and care priorities.2. Performs nursing procedures and interventions, and monitors and evaluates the effectiveness of the procedures and interventions. Procedures include head to toe assessments, monitoring and recognizing abnormal signs or symptoms, including laboratory values, and providing airway management, nutrition management, infusion management and catheterizations. Applies and changes simple sterile and/or nonsterile dressings, provides colostomy care and irrigation, takes/records blood pressures, administers suppositories and enemas and takes/records temperature, pulse and respiration. 3. Consults other members of the interdisciplinary care team as required. Records observations and reports problems and/or changes to designated nursing staff. Supports and partners with the RN in the care of patients with complex or unpredictable outcomes.4. Assesses and monitors client and family’s actual and potential strengths and limitations including physiological, psychological, socio-cultural and spiritual needs. Where appropriate, encourages client participation in activities of daily living. Establishes and maintains a therapeutic relationship with the client and significant others using interpersonal and interviewing techniques, ensuring client choice and autonomy in decision-making and care planning is maintained. 5. Communicates with physician regarding assigned patients and carries out verbal and/or written orders as required. Transcribes orders onto appropriate patient record and processes written orders as needed. 6. Contributes to the development and ongoing changes to the individualized plan of care, including attending and providing input into multidisciplinary care planning meetings and family conferences. Advocates for the interest of the client and family. Administers oral, subcutaneous and intramuscular medications in accordance with organizational and pharmaceutical policies and procedures. 7. Performs peripheral intravenous (IV) catheter insertion. Provides maintenance of IV infusions by assessing the catheter insertion site and monitoring flow rate. Discontinues IV access, as per physician orders. 8. Prepares patient for, and/or receives patient from surgical and/or diagnostic procedures, transfers and admissions. Contacts physician or other member(s) of the interdisciplinary team for relevant patient orders and information. 9. Performs technical skills and provides physician support within the procedure room, in a Team Nursing Approach, upon completion of formal post-basic education and training. 10. Administers medications, including narcotics, excluding intravenous route, according to established procedures. Monitors medication’s effects. Maintains communication with RN or other members of the interdisciplinary team as required. Communicates the client’s condition and changes to the designated RN. 11. Provides personal care to clients including assisting with bathing, dressing, care of skin and hair, changing beds and assisting with toileting needs. 12. Performs admissions, discharges and transfers of clients according to PHC operating policies and procedures. 13. Documents and updates all relevant information about care and procedures to achieve client safety and comfort according to unit specific policy and procedure. 14. Performs related clerical duties such as answering the telephone, faxing, taking and relaying messages, making appointments, ordering supplies, initiating diagnostic requisitions and assisting in maintaining standardized records.15. Assists in the orientation and sharing of information with new staff, students, and other members of the multidisciplinary team. 16. Participates in staff meetings, team conferences, special projects, relevant hospital committees, continuous quality improvement and program evaluation activities. 17. Maintains a safe environment for clients, family and staff, through: a. Accident prevention, b. Knowledge of emergency procedures and regulations, c. Using skills, physical facilities, supplies and equipment safely and appropriately, and d. Practicing infection control procedures. 18. Participates and attends in-service and other educational programs as required. Identifies learning goals. Maintains and updates current clinical competence and knowledge within area of practice. 19. Operates and/or cleans equipment related to patients care and reports malfunctions to supervisor. Uses appropriate supplies for care in accordance with departmental procedures. Handles, stores and disposes of workplace hazardous materials and sharps in accordance with established procedures.20. Performs other related duties as assigned.
