Job Summary

ED: The Registered Staff Nurse is a professional nurse who renders quality nursing care. Nursing care is based upon the assessed needs of the patient, is congruent with the prescribed therapy of the medical staff and is based on the Nursing Department's philosophy of nursing, and the Hospital's mission, vision and values. The Registered Staff Nurse will assess, plan, implement, and evaluate care for all patients. The Registered Staff Nurse is accountable for the delivery of quality patient care, utilizing the nursing process for all patients from birth through their adult life cycle. Professional certification in applicable nursing field(s) preferred. Must be available to work day, evening and/or night shift as well as rotation on weekends as assigned. Must be able to rotate to units as assigned. Must be willing to work additional shifts as assigned. Must be available to stay overnight should need arise during patient transports off island. May be exposed to the following hazards: temperature fluctuations, moderate noise levels, minimal dust, infectious diseases and other chemicals as customarily used in a hospital setting. Exposure to various hazards in handling patient transports. Must be available to work overtime as needed to complete all job duties. Must be available to work in the case of a Hospital declared emergency.

NCMG: The RN is a licensed professional who supports medical providers in delivering of patient and family centered health care in an ambulatory care setting. The nurse reports to the Nurse Director and is responsible for working side by side with the providers, hospital leadership, and health care team to oversee the delivery of quality patient care through the daily operations of the office, which may include the supervision of the medical assistants (MA). Work hours will be determined by the Director of the practice and/or the Clinical Nurse Director and needs of the practice and will include some weekends. Overtime as necessary to complete assigned work. Must be available to work in the case of a declared Hospital emergency. May be required to attend meetings on or off the island.

Perioperative: The Perioperative Nurse assesses the patient preoperatively; plans care during the surgical intervention, implements those nursing actions and evaluate the outcomes. This position is responsible for perioperative nursing, coverage of Central Sterile Supply on an as needed basis, and scheduled outpatient procedures requiring assistance by the physician. National certification in areas of expertise (CNOR, CEN, CCRN, etc.) preferred. Must be available to stay overnight should need arise during patient transports off island. Must be able to rotate to other units, as assigned. Must be willing to work additional shifts, as assigned. Must be available to work in the case of a declared Hospital emergency.

Dialysis: Performs standard nursing practices as outlined by Dialysis Unit policies and procedures. Must be available to work day or evening shifts, as well as weekend rotations, as assigned. Must be available to stay overnight should need arise during patient transports off island. Must be able to rotate to other units, as assigned. Must be willing to work additional shifts, as assigned. Must be available to work in the case of a declared Hospital emergency. Must be tested on hire for color-blindness.

Infusion: The Chemotherapy/IV therapy Nurse functions as an experienced clinical Registered Nurse in coordinating patient care and collaborating with other care providers and health team members to provide high-quality care of patients needing outpatient infusion therapy. This nurse acts as a patient guide and advocate by assisting patients and families to seek information, ensuring informed consent regarding treatment decisions and promoting the maximal level of patient-desired independence. This nurse is accountable for delivering care to the patient within the framework of the nursing process. This is a clinical position assuming department wide responsibility. Must be available to work day, evening and/or night shift as well as rotation on weekends as assigned. Must be able to rotate to units as assigned. Must be willing to work additional/irregular shifts as on occasion to meet patient needs. May be exposed to the following hazards: temperature fluctuations, moderate noise levels, minimal dust, infectious diseases and other chemicals as customarily used in a hospital setting. Exposure to various hazards in handling patient transports. Must be available to work overtime as needed to complete all job duties. Must be available to work in the case of a Hospital declared emergency.

LDRP: This is a Registered Staff Nurse position with responsibility for reasonable and comprehensive care for all obstetrical patients. Accountable for the delivery of quality patient care, utilizing the nursing process for all patients. Must be available to work day, evening and/or night shift as well as rotation on weekends as assigned. Must be able to rotate to units as assigned. Must be willing to work additional shifts as assigned. Must be available to stay overnight should need arise during patient transports off island. May be exposed to the following hazards: temperature fluctuations, moderate noise levels, minimal dust, infectious diseases and other chemicals as customarily used in a hospital setting. Exposure to various hazards in handling patient transports. Must be available to work overtime as needed to complete all job duties. NCMG:

Responsible for handling calls from patients and/or patient representative who are seeking medical information and/or medical attention.  Facilitates scheduling of appointments as the patient’s condition requires, answer questions and provide health education to patients on the phone who have questions about their care.

Specific duties include:

• Participates in daily “morning huddle,” proactively identifying clinical care, concerns, and needs for patient’s/families scheduled for the day.

• Helps facilitate the processing of medication refills.

• Conducts patient call-backs/follow-up calls within a timely manner and documents the interaction within the EHR. 

• Supports the office team with obtaining required medical records/consults/test results.

• Completes medical and treatment pre-authorizations in a timely manner.

• Helps make referral to outside institutions if clinical expertise is needed.

• Assists the providers and other office staff with scheduling patients for required medical appointments, utilizing EMR, and other available scheduling systems.

•  Assists the provider as needed with EMR inbasket management.

• Administers immunizations within established practice guidelines.

• Completion of a pre-visit comprehensive assessment, intake, and history of new patients.

• Rooming patients and preparing the patient and room for the provider, to include the following:

   o Gathering of all appropriate supplies.

   o Opening the clinical record using the appropriate template.

   o Review and documentation of all relevant clinical information in each patient’s records, to include:

• Chief complaint/reason for visit/nursing assessment.

• Current and past medications (medication reconciliation).

• Relevant patient, family, and social history (including smoking status).

• Health Maintenance status/requirements.

• Nursing care provided.

• Vaccine status and administration.

• Vital signs.

• Fall screening for patients >65.

• Review and document allergies.

• Ensure patient’s choice of pharmacy is noted.

• Completion of screening and evaluation forms (PEDS and Post-Partum Screenings).

• Review PROMS questionnaire.

• Confirm all appropriate patient consents for treatment/care (surgical procedures, Care in L&D, Sterilization, etc.) are obtained and help provide education, along with the provider if necessary.

• Ensures all required insurance/forms are reviewed and signed by the patient (Medicare physical and medication/vaccine waivers).

• Comprehensive teaching and education of patients and/or their families utilizing current, standardized teaching tools and handouts.

• Assists the provider with treatments/screenings.

• Completes medication/treatment pre-authorizations.

• Completion of all nurse visits, in a timely manner, to include:

   o Providing and recording appropriate assessment and treatment data.

   o Consults with provider, as required.

   o Completion of coding within the EHR in accordance with documented industry standards and Partners Policy.

• Sends/responds to clinical messages in a timely manner.

• Assists other nurses as needed.

• Keep rooms adequately stocked.

• Assist patients in and out of wheelchairs and other assistance as needed.

• Telephone Triage

   o Responds to patient phone calls in a timely manner, collecting sufficient information related to the presenting problem and medical history.

   o Furnishes medical advice within the scope of the practice.

   o Facilitates the scheduling of appointments, based on necessity.

   o Accurately documents the encounter with the patient in the EMR.

   o Maintains work queue within the EMR.

Perioperative:

• Demonstrates expertise in operating room procedures and can function independently in the performance of other surgical procedures.

• Establishes and implements standards of practice that ensure safe, effective and efficient nursing skills.

• Is able to interpret and complies with JCAHO, OSHA, and other federal and state standards.

• As a perioperative nurse, assesses the patient preoperatively, plans care during the surgical intervention, implements those nursing actions and evaluate the outcomes. Ensures the patients record is complete with all necessary pre-operative information. Documents.

• Conducts a nursing assessment and develops a plan of care for all patients upon admission.

• Identifies and prioritizes activities that are necessary to meet patient needs.

• Evaluates the effectiveness of intraoperative measures taken, documents them and reassesses to take additional actions as needed.

• Communicates and collaborates with other members of the health care team.

• Maintains a procedural log as it pertains to the operating room and other units.

• Covers central supply responsibilities in times of illness/vacation and as needed.

• Ensures that appropriate supplies are available and replaced as used/needed.

• Uses proper aseptic and sterilization techniques.

• Assists in the orientation and supervision of other personnel who work in the perioperative setting.

• Highly recommend CNOR certification within two (2) years of assuming position.

• Oversees the operation of the operating room and central supply in the absence of the Perioperative Services manager.

• Maintains confidentiality of patient information in accordance with Hospital policy and applicable statutes and regulations.

Dialysis:

• Demonstrate understanding of the standards of care related to hemodialysis/ESRD nursing.

• Demonstrates proficiency in performing dialysis nursing by:

   o Assessing patient access for patency and/or complications

   o Performing cannulation efficiently and effectively

   o Assessing patient pre and post treatment

   o performing dialysis treatment per unit standard

   o monitoring for patient response to Rx

   o Assessing patient for discharge

   o Monitoring estimated dry weight appropriately

• Implements, consistently provides and documents patient education related to ESRD/dialysis plan of care in the patient record.

• Reports unusual occurrences to Clinical Coordinator/Clinical Nurse Manager and/or MD as appropriate.

• Demonstrates proficiency in administering treatments, procedures and equipment appropriate to the unit/department as demonstrated by the skills checklist.

• Administers safely and documents medication regimen according to established procedures with no more than three (3) documented errors per year.  

• Uses standard precautions and appropriate personal protective equipment as outlined in the Nursing Department Infection Control Manual.

• Demonstrates competency in the following areas by physical demonstration, verbal or written exam (medical equipment, age specific assessment and intervention, procedures such as IV therapy, lab draws,  medication administration, etc.), as may be required in the performance of job duties and responsibilities.

• Participates in activities designed for professional growth in nursing as well as Nephrology.

• Attends all mandatory Hospital/Nursing Department in-service training sessions and meetings. 

• Participates in department meetings and conferences as required.

• Reviews and signs all in-service updates.

• Identifies own limitations in skills and knowledge as demonstrated by seeking guidance when needed from appropriate colleagues.

• Actively participates in own performance appraisal.

• Helps maintains performance and sterility of dialysis machines by performing:

   o Heat and bleach disinfection

   o Obtaining monthly cultures

   o Maintaining machine logs

• Maintains order by maintaining adequate stock of supplies and ensuring adequate forms for the clinical record, patient education, etc.

• Participates in department quality improvement activities.

• Provides assistance to other staff members when needed and requested.

• Participates in the orientation of new staff members.

• Assists in the scheduling of seasonal/transient patients and obtaining the necessary transfer paperwork.

• Maintains confidentiality of patient information in accordance with Hospital policy and applicable statutes and regulations.

Infusion (1/2):

Assessment: The IV Therapy Nurse systematically and continually collects data regarding the health status of the patient. This data is communicated in a timely manner with other members of the health care team as needed.

• Collects pertinent objective and subjective data in a timely manner, ongoing, systematic, and culturally competent manner.

• Collects data from multiple sources, including the patient, the family, the interdisciplinary cancer care team members, and the community using appropriate assessment techniques.

• Uses theoretical and evidence-based concepts in nursing to assess individual patient populations.

• Collects data in problem areas that may include but are not limited to health promotion, patient/family education, coping, comfort, nutrition, complementary and alternative medicine, protective mechanisms, mobility, gastrointestinal and urinary function, sexuality, cardiopulmonary function, oncology emergencies, end-stage renal disease, psychosocial issues and palliative and end-of-life care.

Diagnosis: The IV Therapy Nurse ensures that the assessment data is analyzed to determine nursing diagnosis.  This nurse is also recognized as the expert in outpatient infusion therapy and used as a resource and mentor to regular nursing staff.

• Determines nursing diagnoses and potential problem statements from assessment data.

• Formulates nursing diagnoses that conform to an accepted classification system.

• Develops individualized nursing diagnoses that are physically, psychosocially, spiritually, and culturally appropriate to the patient.

• Ensures nursing diagnoses reflect the patient’s actual or potential health problems.

• Validates nursing diagnoses with the patient, family, and interdisciplinary care team when possible.

Outcome Identification and Planning: The Nurse ensures that individualized expected outcomes are identified. The IV Therapy Nurse ensures that an individualized and holistic plan of care that prescribes interventions to attain expected outcomes.

• Initiates patient plan of care using assessment data upon admission based on age specific needs.

• Incorporates in the patient plan of care the:

   o Nursing diagnosis specific to the patient’s actual problems/needs.  

   o Nursing diagnosis which reflects complex etiology. 

   o Individual teaching plans using identified teaching needs.  

   o Nursing interventions consistent with nursing diagnosis and etiology.

   o Attainable goals with target dates.

• Documents nursing intervention and patient’s response by the end of the treatments according to established procedures.

• Implements discharge plans by:

   o Making appropriate referrals to other health care services.

   o Implementing individualized teaching plan based on actual discharge needs.

• Formulates the patient plan of care based on acceptable standards of practice.

• Communicates the patient plan of care (written/verbal) through established procedures.           

• Plans individualized patient care using nursing theory in collaboration with patient and other health care providers.

• Implements patient’s plan of care in collaboration with patient, family members and significant others.

• Communicates the initial plan of care and revision (written/verbal) through shift reports and other established procedures.

• Evaluates nursing interventions to changing patient situations as demonstrated by patient outcome, documentation in the patient record and revision of the plan of care.

• Prioritizes nursing interventions in complex and rapidly changing patient situations, as evidenced by the patient outcome and documentation in the record.

• Recognizes acute problems that occur in the unit and intervenes in a safe manner, in accordance with Nursing Department and Hospital procedure.

• The Nurse ensures that the plan of care is implemented to achieve the identified expected outcomes for the patient.

• Administers safely and documents medication regimen according to established protocols with no more than one documented error per year.      

• Uses standard precautions as outlined in the Departmental Infection Control Manual.

• Utilizes learning principles as demonstrated by the documentation of teaching outcomes.

Infusion (2/2):

Evaluation: The IV Therapy Nurse systematically and regularly evaluates the patient’s response to interventions to determine progress toward achievement of expected outcomes.

• Ensures that the patient/family and the members of the interdisciplinary care team participate collaboratively in the evaluation process when possible.

• Maintains a systematic and ongoing evaluation process.

• Collects evaluation data from all pertinent sources

• Compares actual findings to expected findings.

• Reviews and revises the nursing diagnoses, expected outcomes and plan of care based on the findings of the evaluation process.

• Uses evidence-based research in the evaluation of expected outcomes.

• Communicates the patient’s response with the interdisciplinary cancer care team and other agencies involved in the healthcare continuum.

• Collects and documents all data pertinent to each patient’s actual and potential needs within twenty-four (24) hours of admission.

• Documents and reports significant findings of ongoing patient assessment.

• Formulates the patient plan of care based on acceptable standards of practice.

• Communicates the patient plan of care (written/verbal) through shift reports and other established procedures.

• Plans individualized patient care using nursing theory in collaboration with patient and other health care providers.

• Initiates patient plan of care, using assessment data upon admission based on age-specific needs.

• Incorporates in the patient plan of care the:

   o Nursing diagnosis specific to the patient’s actual problems/needs.

   o Nursing Diagnosis which reflects complex etiology.

   o Individual teaching plans using identified teaching needs.

   o Nursing interventions consistent with nursing diagnosis and etiology.

• Implements patient’s plan of care in collaboration with patient, family members and significant others.

• Communicates the initial plan of care and revision (written/verbal) through shift reports and other established procedures.

• Formulates discharge plans within twenty-four (24) hours of admission and as part of the       revision process.

• Implements discharge plans by:

   o initiating and completing discharge screening tool upon admission,

   o completing appropriate referrals to other care services, 

   o implements individualized teaching plan based on actual discharge needs.

• Implements and documents teaching plan in the patient record.

• Evaluates outcome of patient care.

• Evaluates nursing interventions to changing patient situations as demonstrated by patient outcome, documentation in the patient record and revision of the plan of care.

• Prioritizes nursing interventions in complex and rapidly changing patient situations, as evidenced by the patient outcome and documentation in the record.

• Demonstrates proficiency in administering treatments, procedures and equipment appropriate to the unit/department as demonstrated by the skills checklist.

• Administers safely and documents medication regimen according to established procedures with no more than three (3) documented errors per year.  

• Uses standard precautions as outlined in the Nursing Department Infection Control Manual.

• Participates in performance improvement activities by evaluating standards of care and initiating revisions as necessary.

QUALIFICATIONS

Education

Associate's Degree Nursing required. Bachelor's Degree Nursing preferred.

Work Experience

ED: 3 years of prior, related ED experience preferred.

ED: TNCC & NRP preferred.

ED: CPI/AVADE/MOAB training preferred.

Med/Surg, Infusion, LDRP: 1 year of prior, related medical/surgical experience preferred.

NCMG: minimum of 3 years of relevant clinical experience preferred.

Perioperative: at least 2 years of prior perioperative experience

Dialysis: more than 1 year of experience in a medical/surgical unit required.

Dialysis: critical care experience preferred.

Dialysis: ACLS preferred.

Infusion: Strong IV skills or certification

LDRP: at least 2 years of prior labor and delivery experience preferred.
