Contract Certified Home Health / Hospice Aide (CHHA)
Position Overview
We are seeking a compassionate and dedicated Contract Certified Home Health / Hospice Aide (CHHA) to join our team in providing exceptional in-home care to patients requiring assistance with daily living activities and end-of-life care. This contract position offers the opportunity to make a meaningful difference in the lives of patients and their families during critical times, while working independently in various home settings throughout our service area.
As a CHHA, you will be responsible for delivering high-quality, patient-centered care under the supervision of registered nurses and in accordance with established care plans. This role requires a unique combination of clinical skills, emotional intelligence, and the ability to work autonomously while maintaining strong communication with the healthcare team.
Key Responsibilities
Direct Patient Care
- Provide personal care services including bathing, grooming, dressing, oral hygiene, and toileting assistance while maintaining patient dignity and privacy
- Assist with mobility, transfers, and ambulation using proper body mechanics and assistive devices
- Monitor and record vital signs including temperature, pulse, respiration, and blood pressure as directed
- Observe and report changes in patient's physical condition, behavior, or emotional state to the supervising nurse
- Assist with prescribed exercises and range of motion activities as outlined in the care plan
- Provide skin care and implement measures to prevent pressure ulcers
- Assist with medication reminders as permitted by state regulations
Activities of Daily Living Support
- Prepare nutritious meals according to dietary restrictions and patient preferences
- Assist with feeding when necessary, ensuring proper positioning and monitoring for swallowing difficulties
- Perform light housekeeping duties to maintain a safe and clean environment
- Assist with laundry and linen changes
- Accompany patients to medical appointments and assist with transportation when needed
- Provide companionship and emotional support to patients and family members
Hospice-Specific Duties
- Provide comfort care measures for patients in end-of-life stages
- Support family members during the dying process with compassion and professionalism
