Care Coordinator (Full-Time Office Position)

A&C Private HomeCare is seeking a compassionate, organized, and motivated Care Coordinator to join our growing team. This is an in-office position responsible for coordinating caregiver schedules, supporting clients and caregivers, and ensuring exceptional customer service while maintaining compliance with agency policies.

Responsibilities

Coordinate and maintain caregiver schedules.

Match caregivers with clients based on client needs, availability, and caregiver qualifications.

Communicate with clients, caregivers, family members, hospitals, and case managers.

Complete caregiver onboarding and orientation.
-
Assist with recruitment, interviewing, and hiring of caregivers.

Monitor caregiver compliance, credentials, and required documentation.

Conduct routine follow-up calls with clients and caregivers to ensure satisfaction.

Maintain accurate documentation and electronic client records.
-
Assist with call-outs, schedule changes, and emergency staffing needs.

Ensure agency policies, procedures, and state regulations are followed.

Work closely with the management team to promote quality care and excellent customer service.

Participate in team meetings, training, and quality improvement initiatives.

Perform additional administrative duties as assigned.
Knowledge & Skills
-
Knowledge of home care regulations and compliance requirements is preferred.
-
Familiarity with state regulations, Medicaid waiver programs, and Veterans Affairs (VA) requirements is highly desirable.

Understanding of caregiver credentialing, documentation standards, and regulatory compliance.

Ability to maintain confidentiality and ensure compliance with HIPAA and agency policies.

Strong attention to detail with the ability to accurately maintain records and documentation.

Qualifications

High school diploma or GED required (Associate's or Bachelor's degree preferred).

Previous experience in healthcare, home care, scheduling, customer service, or case management preferred.

Strong organizational and time management skills.

Excellent verbal and written communication skills.

Ability to multitask and prioritize in a fast-paced office environment.

Proficiency with Microsoft Office and electronic documentation systems.
