Job Summary and Responsibilities
As our Case Manager RN, you will coordinate the care and services of patients across the continuum of illness; promote effective utilization; monitor health care resources; and assume a leadership role with the interdisciplinary team to achieve optimal outcomes through the tools of case management.
Every day you will perform a wide variety of high-level patient care activities to include admission assessment, screen for appropriateness, and development of a discharge plan. Timely and appropriate discharges will be accomplished by evaluating the patient's post-discharge needs and the provision of services necessary to meet those needs.
To be successful in this role you must be knowledgeable in community resources and alternate care facilities.
- Utilizes the nursing process to provide safe therapeutic care for patients
- Assists with coordination of patient-care activities/scheduling. Collaborates/cooperates to assure effective consistent management of clients
Works collaboratively with patient and family to obtain desired patient outcomes
Obtains all required physician orders, TARs, and authorizations for all services
- Incorporates all State, Federal, and accreditation requirements into care management activities and documentation
- Provides patient and family teaching necessary to meet the needs of the patient and family
Job Requirements
Required
- Minimum of One (1) year of RN experience
- Registered Nurse: CA (RN:CA)
- Basic Life Support - CPR (BLS-CPR) within 30 days
- Driver's License: CA (AUTO:CA)
- Auto Insurance (AUTO-INS)
Preferred
- Hospice or Home Health experience
