The IMPACT Specialist is a specialist-level program implementation and patient navigation professional who supports screening, follow-up, referral, and linkage-to-care initiatives across clinical and community settings. Based on program assignment, the role develops and refines workflows, coordinates with internal teams and external partners, conducts patient outreach and individualized navigation, maintains accurate program data, and uses operational findings to improve access and outcomes. Assignments may include infectious disease, preventive health, chronic disease, cancer screening, health-related social needs, or other priority areas. The Specialist may provide functional direction, training, work assignment, and case review for navigators, students, or other program staff while working under general supervision.Job Description
Essential Functions
Program Implementation and Workflow Development
- Lead or support implementation, maintenance, and continuous refinement of assigned screening, follow-up, referral, and linkage-to-care workflows across clinical or community settings.
- Assess operations, identify gaps or barriers, and collaborate with clinical, operational, technical, and program stakeholders to design and implement practical solutions.
- Build staff understanding and adoption of program processes through relationship building, education, coaching, and responsive troubleshooting.
- Coordinate with IMPACT leadership and other initiative teams to standardize core processes, share effective practices, and support responsible program expansion.
Patient Assessment, Follow-Up, and Navigation
- Review assigned screening results, referrals, test results, or care gaps and determine needed follow-up according to established program criteria and workflows.
- Engage patients to clarify needs, priorities, barriers, preferences, eligibility, insurance or benefit considerations, and other factors relevant to successful connection to services.
- Identify best-fit internal, government, or community resources; implement referrals; schedule or facilitate appointments; assist with required steps or paperwork; and provide ongoing navigation until loop closure.
- Communicate results, referral status, and unresolved barriers within role scope and approved protocols, using established escalation pathways for clinical or safety concerns.
- Manage a structured caseload and document outreach, interventions, referrals, milestones, outcomes, and unresolved needs accurately and promptly.
Data Management, Reporting, and Improvement
- Collect, validate, reconcile, and maintain required program data related to screening, referrals, patient outreach, navigation activities, linkage milestones, service receipt, and outcomes.
- Ensure documentation is accurate, timely, complete, and consistent with privacy, program, organizational, and funder requirements.
- Monitor operational performance and patient outcomes; identify gaps, inequities, delays, unmet needs, or workflow failures; and recommend or implement improvements with program leadership.
- Prepare recurring summaries and contribute to reports, presentations, partner updates, funder deliverables, program evaluation, and quality improvement activities.
Minimum Qualifications
- Associate’s Degree in Public Health, Healthcare Administration or a related field or
- Bachelor’s Degree in Public Health, Healthcare Administration or a related field
- 3 years experience in healthcare or benefits assistance, particularly direct patient care, public health, social work, case management or customer service
- Proficiency in use of Microsoft Word, Excel, Outlook
- Proficiency in data entry and familiarity with electronic health record systems
- Ability to utilize resources effectively to overcome challenges and find solutions
- Ability to multitask, problem-solve, and work in a fast-paced environment
Preferred Qualifications
- Bilingual skills (English/Spanish)
- Prior experience working in a healthcare setting
- Prior experience in community engagement or customer service
Additional Qualifications
- Ability to stand or sit for extended periods
- Ability to perform computer searches and telephonic outreach
- Ability to work in hospital setting
- Comfortable wearing personal protective equipment (PPE) as required by hospital guidelines
- May require weekend or holiday shifts depending on hospital needs
- Requires travel to various on-site locations to conduct screenings, support on site staff/students, troubleshoot workflows, etc.
Physical Demands
Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.

Salary Range
$49,555.65 to $70,616.32 AnnualThe actual hiring rate will be determined based on candidate experience, skills and qualifications. This position is not eligible for an annual incentive.
