Home Health Field LPN – New Port Richey, FL

 

A leading healthcare organization is seeking a compassionate and dedicated Home Health Field Licensed Practical Nurse (LPN) to provide high-quality, patient-centered care in the home setting. Responsibilities

• Provide hands-on nursing care to patients in their homes using established clinical standards and the nursing process

• Deliver care as ordered, including IV infusions, infusion pump management, and tube feedings under the supervision of a Registered Nurse

• Monitor patient conditions and communicate changes in status and treatment responses to the supervising Registered Nurse

• Implement and maintain patient and caregiver education plans under the direction of a Registered Nurse

• Administer medications and evaluate patient responses when appropriate

• Collaborate with interdisciplinary team members to ensure continuity and quality of care

• Maintain accurate and timely documentation of patient care activities

• Promote patient safety, independence, and overall well-being in the home environment

 

Qualifications

• Graduate of an accredited Practical Nursing program

• Current Licensed Practical Nurse (LPN) license in the state of practice

• Current Basic Life Support (BLS) certification

• Valid driver's license and reliable transportation

• Minimum of one year of Licensed Practical Nurse experience required

• Previous home health or home care experience preferred

• Strong communication, organizational, and patient education skills

• Ability to work independently and manage patient visits effectively

• Must meet all applicable background screening requirements
