Home Health Field LPN – New Port Richey, FL
A leading healthcare organization is seeking a compassionate and dedicated Home Health Field Licensed Practical Nurse (LPN) to provide high-quality, patient-centered care in the home setting. Responsibilities

Provide hands-on nursing care to patients in their homes using established clinical standards and the nursing process

Deliver care as ordered, including IV infusions, infusion pump management, and tube feedings under the supervision of a Registered Nurse
-
Monitor patient conditions and communicate changes in status and treatment responses to the supervising Registered Nurse
-
Implement and maintain patient and caregiver education plans under the direction of a Registered Nurse
-
Administer medications and evaluate patient responses when appropriate
-
Collaborate with interdisciplinary team members to ensure continuity and quality of care
-
Maintain accurate and timely documentation of patient care activities
-
Promote patient safety, independence, and overall well-being in the home environment
Qualifications

Graduate of an accredited Practical Nursing program
-
Current Licensed Practical Nurse (LPN) license in the state of practice
-
Current Basic Life Support (BLS) certification
-
Valid driver's license and reliable transportation
-
Minimum of one year of Licensed Practical Nurse experience required

Previous home health or home care experience preferred
-
Strong communication, organizational, and patient education skills
-
Ability to work independently and manage patient visits effectively
-
Must meet all applicable background screening requirements
