Location: Kitchener-Waterloo Region, ON
Salary: $125000 – $147,500 per year
Schedule: Primarily Days | One Evening Shift Every Two Weeks | Flexible Hours

What You'll Do
As a Community Outreach Nurse Practitioner, you will:
- Host and provide care through community-based medical clinics
- Deliver primary healthcare services to individuals across the community
- Conduct home visits when required
- Lead vaccination and immunization clinics
- Collaborate with interdisciplinary teams and community partners
- Coordinate care and liaise with primary care and healthcare resources
- Maintain strict privacy, confidentiality, and professional standards
- Provide compassionate, trauma-informed care with empathy and respect
- Support individuals experiencing barriers to healthcare access

What We're Looking For
The ideal candidate will have:
- Completion of a recognized Primary Care Nurse Practitioner Program
- Bachelor of Science in Nursing (BScN)
- Current registration as a Nurse Practitioner (Extended Class) with the College of Nurses of Ontario
- A valid Ontario driver's licence, reliable vehicle, and automobile insurance
- Experience in primary care and/or community health is considered an asset
- Understanding of the social determinants of health and the impact of social isolation on individual and community well-being
- Excellent interpersonal, communication, and conflict resolution skills
- Strong emotional intelligence and the ability to establish healthy professional boundaries
- Excellent verbal and written English communication skills; additional languages are an asset
- Proficiency with Microsoft Office, email, and other computer applications
- Ability to obtain a satisfactory Police Vulnerable Sector Check
