Clinical Practice Coordinators provides leadership and direction to RN, LPN field staff and Home Care Aides. Assists in developing realistic goal-oriented Care Plans. Participates in selection and orientation of new health care staff. Plans and Presents in-service education programs. Serves as a resource to staff.
Responsibilities
Assessment - Collects pertinent data and information relative to the patient's health or the situation. Recognizes normal and abnormal findings when gathering data., Conducts or delegates a periodic reassessment of client needs and revises the Care Plan as necessary., Coordination of Care - Plans and evaluates care in collaboration with appropriate disciplines., Diagnosis - Analyzes assessment data to determine actual or potential diagnosis, problems, and issues., Evaluates the skills and services provided by health care staff through regular review of client notes, feedback acquired from the client and family, and interaction with staff during case reviews. Assures maintenance of complete, current, and accurate client records., Evaluation - Evaluates progress toward attainment of goals and outcomes. Reports data and outcomes to others as appropriate., Implementation - Implements care or work plan in alignment with the plan and approved safety, infection control, and department/organization standards., Knowledge - In collaboration with leaders, actively pursues required knowledge and skills through orientation activities specific to the position, reading current literature and seeking new learning opportunities., Maintains a relationship with referral sources and community resources., Oasis C Certification, ICD coding knowledge., Other duties as assigned to assist with overall operations of the agency., Outcomes Identification - Identifies expected outcomes for a plan individualized to the patient or situations., Oversees scheduling of staff for home health visits., Participates in local professional organizations and activities to stay current with new technologies or developments and to promote the LHC in the marketplace., Participates in planning and presenting regular in-service education programs to keep staff current in changing health care procedures and government regulations. Maintains records of in-services presented and employee attendance to assure any related government requirements are met., Participates in regular Utilization Reviews as required. Completes weekly chart audits to insure accuracy of frequencies, regulatory compliance., Participates in the selection and orientation of new health care staff, assuring compliance with applicable government regulations for certification or competency testing., Performs or delegates the initial evaluation of client needs., Planning - Develops a plan that prescribes strategies to attain expected, measurable outcomes., Provides counsel in the interpretation of nursing standards and company policies and procedures to all levels of health care staff., Provides direct patient care if needed., Provides input in the implementation of disciplinary, probationary, or termination procedures., Relationships - Establishes effective working relationships with peers, physicians, and other members of the health care / work team.
