Join a new grant-funded program that connects patients leaving the emergency department with community resources to help manage chronic health conditions (i.e. high blood pressure, cardiovascular disease, diabetes, obesity). You’ll work with team members to support patients as they transition from emergency care to ongoing education, services, and support in the community. This may include assisting clients with understanding their conditions, helping access health insurance, following care plans, accessing medications and healthcare services, addressing social and environmental barriers, and connecting with community education resources. Responsibilities may involve client case management including assessment, planning, advocacy, follow-up, and home visitation.
Applicants with diverse backgrounds and bilingual skills are encouraged to apply.
Minimum Qualifications
Graduation from high school or equivalent and six months of experience with local, state, and federal assistance programs including health care and social service systems; or any equivalent combination of training and experience that provides the desirable knowledge, abilities, and skills.
Necessary Special Requirements
Possession of a valid driver’s license when operating a vehicle is necessary to the satisfactory performance of assigned duties.
Evening and limited weekend hours are possible; alternative work schedule will involve hours from 11 a.m. to 7:30 p.m.
