Description
Catawba County Social Services seeks a Social Worker II – Home Delivered Meals Coordinator to join the Senior Nutrition Services team. In this role, you will coordinate the Meals on Wheels Program, provide social work and case management services, and provide backup support for the Volunteer Coordinator. If you are a collaborative, detail-oriented, and motivated Social Worker with a passion for working with older adults and experience in large program coordination, we'd love to hear from you.
- Coordinates and monitors the Meals on Wheels program to ensure compliance with the Home and Community Block Grant Standards and other health requirements
- Works with caterers to ensure quality food and service (food temperature, etc.)
- Provides training to site supervisors on Meals on Wheels responsibilities
- Develops delivery routes to ensure meals are delivered safely and efficiently
- Reviews temperature sheets and other nutrition documentation for accuracy
- Schedules monthly temperature checks for all meal delivery routes
- Handles reporting and other documentation
- Serves as backup and assists the Volunteer Coordinator as needed
- Conducts home visits
- Completes assessments and determines eligibility
- Works with GIS staff to troubleshoot problems with the mapping program
- Conducts a monthly internal audit of the home-delivered meal record with staff and the supervisor to ensure compliance with standards
- Reviews staff performance reports to ensure data accuracy
- Bachelor’s degree from an accredited school of social work; OR
- Bachelor’s degree in a human services field and 1 year of social work experience; OR
- Bachelor’s degree in a non-human services field and 2 years of social work experience
- Valid driver’s license with a safe driving record (Driving is an essential function of this position)
- Excellent customer service and communication skills with an outgoing personality
- Ability to work collaboratively with team members and community partners
- Strong problem-solving and analytical skills
- Knowledge of the delivery area and ability to develop maps for efficient delivery routes
- Proficient computer skills
- This position is considered light work; may lift up to 20 pounds to carry meals and coolers.
