JOB SUMMARY
The Diabetes Educator provides individualized and group education to help patients effectively manage diabetes. This role develops personalized care plans, provides guidance on blood glucose monitoring, medications, nutrition, and physical activity, and supports patients in achieving their health goals. The Diabetes Educator monitors patient progress, collaborates with the healthcare team to ensure comprehensive care, and maintains current knowledge of diabetes management guidelines and best practices.
SPECIFIC ELEMENTS AND ESSENTIAL FUNCTIONS
- Conduct individual and group diabetes education sessions
- Develop personalized care plans for patients with diabetes
- Provide instruction on self-monitoring of blood glucose levels
- Teach patients how to administer insulin and other diabetes medications
- Offer nutritional counseling and meal planning guidance
- Educate patients about the importance of physical activity and exercise
- Assess patients' knowledge and skills related to diabetes management
- Support patients in setting and achieving diabetes-related goals
- Monitor and document patients' progress over time
- Collaborate with other healthcare professionals to ensure comprehensive care
- Stay up-to-date with the latest diabetes management guidelines and research

Requirements
EXPERIENCE
Required:
- Experience working with diverse patient populations; bilingual/multilingual preferred
- Experience developing and delivering patient education programs
Preferred:
- 2–3 years of experience in diabetes education or related field
EDUCATION
- Certified Diabetes Educator (CDE) certification or eligibility
- Bachelor’s or advanced degree in nursing, nutrition, or related field
- Active RN, Registered Dietitian (RD), or Pharmacist licensure
- Completion of a recognized diabetes education program
CERTIFICATIONS/SKILLS
- Strong knowledge of diabetes management, treatment, technologies, and devices
- Strong communication, interpersonal, public speaking, and group facilitation skills
- Proficiency with electronic health record (EHR) systems
- Strong organizational, time management, problem-solving, and critical-thinking skills
- Ability to work independently and collaboratively
- Commitment to patient-centered care and continuous improvement
