POSITION PURPOSE


The Regional Director of Community Health & Well-Being (CHWB) works in collaboration with the VP of Population Health to promote optimal health for those populations who are poor and vulnerable in the communities we serve by connecting social and clinical care, addressing social needs, dismantling systemic racism and reducing health inequities. This position is responsible for leading, developing and overseeing all aspects of the CHWB strategy (investing in our communities, advancing social care and impact the social influencers of health), with a focus on bridging relationships with the Clinically Integrated Network and Medical Group as well as developing programs and outreach strategies in communities throughout the regional service area. This position leads community-based functions including community health workers, community benefit compliance and reporting, and grant development and implementation.

LOCATION:

Statewide (Iowa)This role could sit in either Des Moines, Iowa; Mason City, Iowa; or Davenport, Iowa.

ESSENTIAL FUNCTIONS


1. Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions.


2. Directs the development and implementation of a framework to connect social and clinical care across the region, including the development of a community health worker infrastructure. This framework includes the coordination and integration of community health workers and care managers to ensure collaborative and effective care for people who are dually enrolled in Medicare and Medicaid and other vulnerable populations we serve.


3. Advances partnerships to develop a seamless system of social care support with multiple partners including Regional Health Ministry Hospitals and Ambulatory leaders, government, academic institutions, payers, community-based organizations, and faith-based organizations.


4. Oversees the development and implementation of policies, procedures, tools and training across Regional Health Ministry to ensure compliant, comprehensive and accurate reporting of community benefit activities as required by IRS regulations.

5. Oversees analysis of the healthcare landscape to identify potential opportunities for advancement of CHWB, inclusive of anchor strategies (local hiring, local sourcing, and local investing).


6. Brokers internal partnerships (i.e. safety net clinics, advocacy, mission, clinically integrated network, medical group, finance, marketing/communications, supply chain, etc.) to align CHWB strategy with our mission and core values, other departmental goals and messages.


7. Participates as the CHWB representative on the Regional Integrated Clinical Leadership Team.


8. Leads the region in addressing social influencers of health, through development of policies, systems and environmental change strategies that are community-based and community partnered initiatives/innovations to promote health equity and dismantle systemic racism.


9. Leads and is accountable for CHWB department and programming across a region. Interviews, hires, orients, oversees training and development, schedules, assigns work to, evaluates the performance of and, when necessary, disciplines, all direct reports. Ensures compliance with established personnel policies when operating the Department.


10. Develops long and short-term goals, objectives, plans, quarterly forecasting and annual community benefit budgets for all of CHWB to ensure continued quality of services through planning across the region while also ensuring CHNA implementation efforts are resourced.


11. Serves as a CHWB resource and convener of stakeholders, representing Regional Health Ministry's commitments to community health improvement and partnerships.


12. Develops measurable community health improvement plans and implementation strategies, in collaboration with Ministry senior leadership, that align with the community health needs assessment and Trinity Health strategic plan.


13. Identifies and establishes rapport with funding sources such as foundations, state and federal agencies and establishes reporting systems for each funded initiative in order to ensure that programmatic designs comply with all applicable rules and regulations.


14. Represents Regional Health Ministry at professional or community organization meetings and on committees at local, state and national level in order to assist in developing new programs and policies for improved delivery of health services those who are poor and vulnerable.


15. Oversees the development and implementation of a Community Health Needs Assessment (CHNA) every three (3) years in partnership with community stakeholders and residents, placing particular emphasis on the needs of those who are poor and vulnerable and addressing social influencers of health.


16. Presents CHWB information to the RHM Board, at least bi-annually, to ensure alignment with regional Ministry strategy, support for CHWB activities and compliance with corporate and government requirements.


17. Directs and participates in grant making process and charitable contributions to the community.


18. Ensures opportunities for professional development for collective learning and understanding for internal stakeholders including program staff and leadership.


19. Maintains a working knowledge of applicable Federal, State, and local laws and regulations, Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical, and professional behavior.


MINIMUM QUALIFICATIONS


1. Master’s degree in Health Care Administration, Public Health, Community Health or related field required.


2. At least 10 years of experience in a leadership level/supervisory capacity with proven strategic planning, fiscal management and leadership capacity.


3. Must be able to operate effectively in a collaborative, shared leadership environment. Possesses a personal presence that is characterized by a sense of honesty, integrity

and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals and values of Trinity Health.


4. Exceptional leadership, management and facilitation skills. Demonstrated ability to skillfully counsel, collaborate and negotiate; inspire and build confidence in others; create alliances; obtain support and respect from diverse groups and foster an understanding and commitment to the organization. Ability to adapt style and approach to match the needs of varying teams, individuals and situations.


5. Excellent oral and written communication, persuasion, and negotiation skills necessary in order to facilitate communication between diverse groups; to effectively identify, assess and facilitate improvements and resolutions, and to present advisory, persuasive and authoritative recommendations to all levels of the organization.


6. Excellent program design and evaluation skills.


7. Ability to manage the employees within assigned unit/department and to persuade and negotiate with peer level director and department heads on issues and programs that impact assigned unit/department.


8. Excellent analytical, problem solving and organizational skills to understand best practices, translate them into viable solutions, create value propositions, metrics and practices and to organize and prioritize multiple projects and requests.


9. Ability to operate in an ambiguous and highly matrix organizational structure. Ability to operate in a highly autonomous self-directed manner under frequently changing structures and requirements and work priorities.


10. Must be able to operate effectively in a collaborative, shared leadership environment.


11. Must possess a personal presence that is characterized by a sense of honesty, integrity and caring with the ability to inspire and motivate others to promote the philosophy, mission, vision, goals and values of Trinity Health.
