In this role, you will guide new and existing patients through a smooth enrollment process into our care management programs.
Chronic Care Management (CCM)
CCM stands for Chronic Care Management, which is a healthcare delivery model aimed at providing comprehensive and coordinated care to patients with chronic conditions. CCM services are designed to improve the quality of care, enhance patient outcomes, and reduce healthcare costs by addressing the complex needs of individuals living with chronic illnesses.
What you’ll do:
- Connect with eligible patients
- Educate and enroll patients into preventative care programs
- Guide patients through smooth onboarding process
- Accurately manage documentation

Qualifications:

- High school diploma or equivalent
- Healthcare experience preferred
