Clinical Team Lead
In this key leadership role, you'll help guide a multidisciplinary team of clinicians, support high-quality patient care, and make a meaningful impact on the lives of patients and families in the comfort of their homes.

What You'll Do
As the Clinical Team Lead, you will:
- Provide clinical leadership and support to nurses, aides, therapists, and social workers
- Coordinate patient care, clinician assignments, and referrals
- Oversee care plans to ensure quality outcomes and regulatory compliance
- Review clinical documentation and support timely billing and reimbursement processes
- Monitor high-risk patients and collaborate with clinicians to reduce hospitalizations
- Conduct chart audits and participate in quality improvement initiatives
- Serve as a clinical resource and mentor for field staff
- Support staff onboarding, education, and competency development
- Step into leadership responsibilities in the absence of the Clinical Director/Manager
What We're Looking For
- Active Ohio RN or LPN license
- Minimum 3 years of clinical nursing experience
- At least 1 year of home health experience required
- Previous supervisory or leadership experience preferred
- Strong communication, critical thinking, and organizational skills
- Valid driver's license and reliable transportation
