Description
Under the direction of the Program Manager, the Case Manager will provide compassionate, client-centered case management services, engaging individuals experiencing homelessness, connecting them to resources, and supporting them in achieving meaningful outcomes and greater stability.

Qualifications, skills and experience required:
• Ability to understand and practice the mission of Camillus House.
• Education: Bachelor’s Degree preferred in Social Work, or related field any appropriate combination of education, experience and/or certification will be considered
• Experience: A minimum of 2 years of experience in the same or similar job role is preferred.
• Language: Proficiency in oral/written English; Spanish preferred; additional languages a plus (i.e., Creole).
• Skills: Proficiency in computer (Word, Excel, other data collecting programs).
• Successful completion of Toxicology test, L2 fingerprinting, local background screening, and OIG reference check.
• Valid FL Driver License with a clean driving record.

• Develop individualized housing stability plans in collaboration with client addressing short-term and long-term goals.
• Be able to understand and practice the mission of Camillus House.
• Be comfortable working directly with highly vulnerable populations to complete thorough needs assessments, work with their assigned clients to develop service plans and goals to address their unique needs and coordinate with inside and outside partners to secure services for the client (e.g. benefits, transportation, training, training, etc.), with the goal of addressing issues and barriers so that they may secure and/or maintain housing stability.
• Be proficient in oral/written English and have the ability to document client progress and activity
• Have competency in MS Office/Outlook and other data entry programs
• Has prior experience providing case management to vulnerable populations and the homeless in particular. For Case Managers in Behavioral Health programs (mental health and substance abuse), prior experience working with those populations, or specific training/certifications in those areas, is preferred.
• Is familiar with the homeless system of care (e.g. coordinated entry, housing first, HMIS)
• Develops individualized housing stability plan in collaboration with client addressing short-term and long-term goals.
• Is familiar with other services available to assist homeless persons, (Social Security, DCF, Medicaid, etc.)
• Is familiar with evidence-based models in a social work setting.
Work Schedule:
- Full-time: Monday through Friday
- Ability to work a flexible work schedule (i.e., Evenings, weekends, holidays, and on-call), as needed.
