SCHEDULE/HOURS: M-F 8 - 4:30pm
REPORTING STRUCTURE: Reports to the Care Center Administrator, Clincal Administrator and / or Campus Administrator

Responsibilities
The Admissions Nurse (LPN) is responsible for enhancing patient and resident satisfaction by facilitating the nursing admission process. This includes collaborating with RNs and other healthcare professionals to assess appropriateness of admissions, assisting with coverage determinations, coordinating care with the multidisciplinary team, supporting discharge planning, and participating in utilization reviews for various payer types. The LPN ensures smooth transitions and adherence to care standards, contributing to efficient and high-quality patient outcomes.
Qualifications
- Licensed Practical Nurse with current licensure with the State Board of Nursing in state(s) in which he/she practices.
- Previous admissions/or intake experience preferred. Long term care, acute care or subacute experience preferred.
- Knowledge of MDS, payer systems and contracts (Medicare, HMO’s, Managed Care, Third Party Payer Systems)
- Demonstrated organizational skills and abilities to handle multiple priorities accurately and time to facilitate customer and program needs.
- Experience in human relations, sensitivity to persons in crisis, good listening skills. Ability to handle inquiries and relate well with referral sources, prospective patients/residents and families, and to handle confidential information and admission records
- Must be able to work flexible hours to meet customer needs, including but not limited to weekdays, weekends and evenings.
- Demonstrated compatibility with PHS's mission and operating philosophies.
- Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
Pay Range
The estimated starting pay range for this role is $30.76 - $32.99 per hour. This good faith estimate is based upon PHS’s typical starting pay range for this role. Actual pay range depends upon many factors including each individual employee’s qualifications and market conditions and is subject to change.Benefits
