We have an exciting opportunity for a In-Reach Recovery Coordinator who has the skills and experience to assess and engage with our stakeholders.
Where: Nottinghamshire (County South and Bassetlaw for the bases and the role covers whole county)
Hours: Full time, 37.5 hours per week
Contract Type: Fixed Term Contract until 31st March 2028
Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
Responsibilities
About the role:
With this role you’ll be working as part of Nottinghamshire County Rough Sleeping Team (RSDAT)
The team supports people with their drug and/or alcohol issues identified as SMD (Severe Multiple Disadvantaged) in accommodation based settings.
The In-Reach Recovery Coordinator will provide specialist drug and alcohol support to residents identified as SMD (Severe Multiple Disadvantage) living within accommodation based settings. The role aims to improve engagement with treatment services, reduce substance-related harm and support individuals to sustain accommodation and improve overall wellbeing.
The post holder will work alongside accommodation providers, housing teams, healthcare professionals, treatment services, and other partner agencies to provide person-centred, trauma-informed support to individuals experiencing substance misuse alongside multiple and complex needs. As well as upskilling and providing training for accommodation providers.
In addition, the post holder will:
- Deliver a specialist drug and alcohol in-reach service within accommodation-based settings for individuals experiencing Severe Multiple Disadvantage (SMD).
- Engage and support individuals who face barriers to accessing mainstream treatment services, using trauma-informed, strengths-based, and psychologically informed approaches.
- Provide brief interventions, harm reduction advice, overdose prevention education, and Naloxone training to both service users and accommodation providers.
- Assess substance misuse needs, identify barriers to engagement, and facilitate access to appropriate treatment, recovery, and healthcare services, including CGL and other specialist providers.
- Develop positive working relationships with accommodation providers and partner agencies to improve outcomes for residents with complex needs.
- Provide guidance, awareness sessions, and informal training to accommodation based settings on substance misuse, harm reduction, recovery pathways, and supporting people experiencing SMD.
- Participate in multi-agency meetings, safeguarding processes, and joint risk management arrangements to promote coordinated support.
- Maintain accurate records and contribute to the monitoring and evaluation of service outcomes, ensuring compliance with organisational policies and procedures.
About you:
- Have a strong understanding of substance misuse issues and/or experience of working within a similar field
- Be an excellent communicator, both verbal and written with good IT skills
- Proactive and able to work on own initiative with excellent time management and prioritising skills
- Be a supportive team player with strong interpersonal skills with the ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
- Ability to manage change successfully in a way that prioritises the needs of service users
- Understand the importance of information governance processes and commit to follow and apply all necessary safeguards
- Seek out learning opportunities to improve and broaden your professional knowledge and skills and to contribute and oversee the learning and development of others
