Pay Range:

$63.45 - $95.20

Oversees all aspects of the clinical triage services, disease management, clinical discharge call management, clinical patient navigation, and virtual care coordination. Supports strategic development and implementation of a patient navigation center. Provides virtual coordination of care and clinical integration (CI) within CI Information Services and others in the outpatient setting.

Essential Functions

• Supervises all clinical activities including; telephone nurse triage, disease management, virtual patient navigation, virtual care coordination of remote team members and clinical components of discharge call management programs.
• Ensures information systems are in place that support the input and retrieval of reliable data.
• Develops, implements, and evaluates department's care delivery models.
• Improves data collection elements, protocol development, and overall delivery of virtual nursing care.
• Develops and assumes accountability for the management of the department budget and guidelines.
• Facilitates reports on all clinical services provided by designated departments.
• Develops and implements a plan or contracts of agreement between multidisciplinary health care, service providers, community resource providers, and allied healthcare providers to facilitate seamless continuity of care throughout the healthcare setting.
• Establishes organizational systems that ensure implementation strategies consistent with established plans: facilitating shared governance and employee engagement in order to effectively operationalize system-wide strategies.
  

Physical Requirements
Work requires walking, standing, and long periods of sitting at a computer work station, utilizing a headset. Work requires lifting up to 30 pounds. Must speak and understand English fluently. Intact sense of sight, hearing, smell, and touch. Good finger dexterity for computer entry. Critical thinking ability to concentrate in a fast-paced nurse triage environment. Must be able to respond quickly to changes in telephone triage unit demands and conditions.

Education, Experience and Certifications

Current RN license or temporary license as a Registered Nurse Petitioner in the state in which you work and reside or; if declaring a National License Compact (NLC) state as your primary state of residency, meet the licensure requirements in your home state; or for Non-National License Compact states, current RN license or temporary license as a Registered Nurse Petitioner required in the state where the RN works. MSN required; If undergraduate degree is BSN a Master's degree from a health-related program or MBA is acceptable. Must have MSN if undergraduate degree is not BSN. Appropriate professional leadership certification required within 1 year of eligibility for professional certification exam required. Previous leadership experience preferred. Additional education, training, certification, or experience may be required within the department by the nurse leader.
