JOB PURPOSE AND REPORTING STRUCTURE: Under the direction of the Community Support Program (CSP) Director, the Nurse administers injections as directed by the psychiatrist or APNPs and coordinates medication administration for all CSP clients through communication with doctors, CM and charting system, in accordance with agency policies and procedures. Work as part of a dynamic, integrative team of mental health professionals with the goal of helping clients maintain their independence in the community.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Administer injections in the CSP and if necessary and at patient homes.
- Chart all work done on behalf of the client.
- Coordinate medication lists in charts.
- Order supplies as needed.
- Communicate with CSP staff as needed to ensure appropriate client treatment.
- Participate in the planning and staffing as appropriate.
- Provide routine medical care under supervision related to provisions of mental health services.
- Complete special projects as assigned.
- Complete and maintain all required paperwork, records, documents, etc.
- Follow and comply with all safety and work rules and regulations. Maintain departmental housekeeping standards.
MARGINAL JOB FUNCTIONS: Maintain client charts. Follow up with clients who miss appointments.
Assist in scheduling follow-ups as necessary. Complete ROI as needed.
Coordinate and track lab usage.
Review medical consents with Case Managers.
PROTECTIVE CLOTHING/EQUIPMENT REQUIRED: Requires the use of protective gloves and a sharps box for disposal of syringes.

EDUCATION and/or EXPERIENCE: Requires a bachelor's degree in nursing and two to three years of related experience; or a combination of education and experience.
