Summary :

The Documentation Integrity Specialist is responsible for supporting the integrity, accuracy, and compliance of the legal health record through management of patient amendment requests, HIPAA-related operational functions, and documentation integrity processes. This position serves as a liaison between patients, providers, HIM, Compliance, and operational stakeholders to ensure timely handling of requests, adherence to regulatory requirements, and maintenance of accurate patient records.
The Documentation Integrity Specialist monitors and tracks amendment requests and other privacy-related workflows, facilitates provider review and decision-making, performs record updates as appropriate, and escalates concerns to HIM Leadership, Privacy, or Compliance teams when necessary. This position plays a critical role in protecting patient rights, supporting regulatory compliance, and maintaining the integrity of the medical record.

Job Description:

JOB RESPONSIBILITIES:

  • Manage patient medical record amendment requests from receipt through final resolution.
  • Maintain departmental logs and metrics related to amendments, privacy requests, and documentation integrity workflows.
  • Assist with patient rights requests related to amendments, restrictions, and accounting of disclosures.
  • Support operational HIPAA privacy functions within HIM.
  • Serve as the primary HIM contact for providers regarding amendment and documentation integrity requests.
  • Collaborate with HIM Leadership, Privacy, Compliance, Clinical Operations, and other stakeholders to resolve issues.
  • Assist with chart corrections, identity integrity activities, and documentation-related workflows.
  • Support initiatives designed to improve medical record accuracy and integrity.
  • Maintain confidentiality of protected health information (PHI) at all times.
  • Participate in special HIM projects and operational initiatives as assigned.
  • Back up to the Documentation Integrity Auditor for Provider Suspension process flows
  • Cross training for chart deficiency analysis and other medical record process flows as needed/required

QUALIFICATIONS:

  • High School Diploma or equivalent required.
  • CRCR, Associate degree in Health Information Management, Healthcare Administration, or related field required.
  • Experience in Health Information Management, medical records, compliance, privacy, documentation integrity, or related healthcare operations preferred.
  • Experience working with electronic health records (Epic preferred).
  • Experience managing amendment requests, chart corrections, release of information, privacy requests, or documentation integrity processes preferred.
  • Experience working directly with providers, clinical staff workflows preferred.
  • Experience with regulatory or compliance-related healthcare processes preferred.
