Job Posting: Physician - Long Term CareJob DescriptionLTHC Solutions, LLC is actively seeking a qualified and dedicated Physician (MD/DO) to serve in a Full-Time role within our innovative long-term care practice. This opportunity is ideal for professionals who are deeply committed to the medical care of older adults across skilled nursing and post-acute care settings. This is not a remote position and will require onsite presence in multiple facilities across Texas. Since our establishment in 2015, LTHC Solutions has been at the forefront of integrating high-quality healthcare by partnering with skilled nursing facilities. We incorporate a holistic approach with advanced technological support, including onsite physician services, telemedicine, Remote Patient Monitoring (RPM), and AI-enhanced clinical communications. As a key member of our team, you will foster better clinical outcomes, enhance the quality of life for vulnerable patient groups, and reduce hospital readmissions through dedicated, compassionate, and evidence-based healthcare delivery.The right candidate for this role will be responsible for delivering comprehensive primary care, managing broad aspects of resident healthcare, and collaborating effectively with an interdisciplinary team to ensure the delivery of outstanding, resident-centered care.Duties and ResponsibilitiesProvide comprehensive primary care services for approximately 150–200 residents across multiple skilled nursing and long-term care facilities.Perform admission history and physical examinations, annual wellness visits, federally required regulatory visits, and medically necessary follow-up evaluations.Develop and manage individualized treatment plans for both acute and chronic medical conditions.Evaluate residents following hospital discharges to guarantee safe transitions of care.Review and interpret laboratory, imaging, and diagnostic studies; implement appropriate treatment plans based on these findings.Maintain accurate, timely, and CMS-compliant clinical documentation.Participate in the advance care planning, engage in goals-of-care discussions, and foster hospice collaboration.Coordinate specialty referrals and interdisciplinary care as necessary.Conduct weekly and/or bi-weekly facility rounds to oversee and manage patient care effectively.Support facility compliance with CMS regulations and involve yourself in quality initiatives.Educate and train nursing staff on the management of complex chronic conditions.Provide clinical guidance aimed at minimizing unnecessary emergency room visits and hospital readmissions.Utilize advanced technology platforms from LTHC Solutions for efficient, proactive care delivery through tools like LTHCareline Telemedicine, PlatinumLine AI Clinical Communication, and Remote Patient Monitoring.RequirementsDoctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree is essential.Active, unrestricted medical license in Texas, or eligibility to obtain one.Current DEA registration required.Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.Must be able to travel between assigned facilities throughout Texas.Willingness to participate in a shared on-call schedule, particularly related to controlled substance and narcotic management.Prior experience in skilled nursing facilities, long-term care, post-acute care, or geriatric medicine highly preferred.Familiarity or experience with telemedicine and value-based care models will be considered an advantage.Knowledge of CMS regulations, survey process, and quality initiatives preferred.Experience with electronic medical records systems such as PointClickCare.Leadership experience in long-term care quality improvement or medical staff collaboration is advantageous.BenefitsEligible full-time physicians receive a comprehensive benefits package, including:Company BenefitsHealth InsuranceDental Insurance401(k) Retirement PlanFlexible Scheduling
