The Care Management team at Indian River Hospital coordinates medical services, plans safe discharges and manages complex or long-term health conditions for its patients. By joining this team as an Assistant Nurse Manager (ANM), you will utilize your clinical expertise and understanding of care management and Medicare regulations to contribute to the goals of cost containment and quality care and provide safe and appropriate transitions of care. Your efforts will help to reduce re-admission rates and length of stay. In this role, you will find the opportunity to take part in an ANM Residency Program as well as potentially move into other leadership positions down the line.
A caregiver in this position works Monday-Friday from 8:00AM – 4:30PM.
A caregiver who excels in this role will:
Coordinate and facilitate patient care progression throughout the continuum.
Collaborate with Social Work on the facilitation of transfers internally to SNF, ARF, LTACH and Home Care.
Work collaboratively and maintains active communication with physicians, nursing, and other members of the interdisciplinary team in order to effect timely and appropriate patient management.
Assist in the development of departmental practices and procedures for transitional care planning ensuring compliance and Medicare regulations are met.
Analyze productivity and other outcome measures.
Identify opportunities for continuous improvement.
Manage information systems to maximize the efficiency of review and transitional care planning.
Develop and monitor routine data quality validation.
Act as a liaison with other review-related and transitional care planning related functions and communicate with departments as necessary to ensure coordination of related processes and functions.
Minimum qualifications for the ideal future caregiver include:
Bachelor’s Degree in Nursing (BSN)
Current and valid license to practice as a Registered Nurse (RN) in the State of Florida
Care Management Certification
Two years of recent experience in care management or transitional care planning
Three years of experience in hospital-based nursing
Preferred qualifications for the ideal future caregiver include:
Master’s Degree in Nursing (MSN) or related field
Certification in Care Coordination and Transition Management (CCTM), Accredited Case Manager (ACM) or Certified Case Manager (CCM)
Previous supervisory experience
