Ambulatory Pharmacy Technician Specialist

The Ambulatory Pharmacy Technician Specialist supports ambulatory pharmacy services by working collaboratively with ambulatory care pharmacists and clinic teams to assist with patient outreach, navigation, medication access coordination, pre and post visit management, medication history support, adherence initiatives, and program operations. This role helps identify patients who may benefit from pharmacy support, coordinates follow-up, documents care coordination activities and escalates medication-related concerns to the appropriate pharmacist or care team member.

Job Description:

Essential Responsibilities:

1. Works collaboratively with ambulatory care pharmacists, clinic teams, providers, specialty pharmacies, medication access teams, payers, and other stakeholders to support pharmacy-led patient care and care coordination across multi-site specialty programs.

2. Reviews patient lists, specialty prescribing registries, referral queues, work queues, dashboards, and reports to identify patients eligible for pharmacy outreach, adherence support, medication access assistance, population health interventions, or pharmacist follow-up.

3. Performs direct patient outreach by phone, MyChart, and other approved communication channels to support program enrollment, pharmacist appointment scheduling, post-visit start confirmations, lab reminder outreach, annual medication access check-ins, and resolution of non-clinical barriers to care.

4. Assists ambulatory care pharmacists with medication management support workflows, including gathering medication histories, collecting medication-related information for MTM visit preparation, identifying patient-reported adherence barriers, and escalating clinical concerns to the appropriate pharmacist.

5. Supports medication access navigation by gathering prescription insurance information, identifying affordability or coverage barriers, assisting with patient assistance documentation, and proactively coordinating next steps with the appropriate team.

6. Answers, triages, and routes patient or clinic inquiries related to ambulatory pharmacy services, medication access, specialty pharmacy coordination, scheduling, program participation, prior authorization status, and follow-up needs.

7. Maintains program trackers, dashboards, reports, and key performance indicators to support monitoring of outreach volume and program goals.

Required Qualifications:

1. High School diploma or GED required.

2. 3-5 years of related work experience required in pharmacy, ambulatory clinic operations, medication access, population health, managed care, or specialty pharmacy

3. Pharmacy Technician Registration or Licensure in good standing with the Massachusetts Board of Registration in Pharmacy required.

4. National Pharmacy Technician Certification

5. Must have or obtain PTCB recognized Medication History certification within 18 months of hire.

Preferred Qualifications:

1. CPhT-Adv certification.

2. Experience supporting medication adherence initiatives, quality metrics, or care gap closure.

3. Familiarity with Epic, MyChart, referral queues, patient outreach work queues, registries, dashboards, or program tracking tools.

4. Experience with prior authorization support, prescription benefit navigation, financial assistance programs, copay cards, grants, manufacturer assistance programs, or pharmacy benefit coordination.

5. Motivational Interviewing training or experience with patient-centered outreach preferred.

6. Knowledge of state and national prescription assistance programs preferred.

Independence of Action: Ability to organize daily work, manage assigned outreach and follow-up tasks, and complete responsibilities within general guidelines. Work progress is monitored by ambulatory care pharmacists, supervisors or managers.

Written Communications: Ability to communicate clearly and effectively in written English with internal and external customers. Accurately documents patient communication, care coordination activities, and follow-up needs.

 

 

Pay Range:

$32.00 - $43.07

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.  Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
