About The Role:Reporting to the Manager, Community Programs, the Nurse Coordinator plays a key role in the coordination, delivery, and ongoing management of Southlake Health's established Southlake@home programs. Working collaboratively with patients, families, caregivers, physicians, interprofessional hospital teams, Ontario Health @Home (OH@home), and contracted community service providers, the Nurse Coordinator identifies and assesses eligible patients and facilitates safe, timely transitions from hospital to home.
The Nurse Coordinator serves as a central point of contact between the hospital, patients, families, and community care partners, ensuring seamless communication, coordinated care planning, and continuity of services throughout the patient's participation in Southlake@home. The role supports multiple specialized hospital-to-home pathways, providing individualized, patient- and family-centred care that responds to increasingly complex clinical and functional needs.
The Nurse Coordinator is responsible for facilitating referrals, assessing program eligibility, coordinating discharge planning, confirming appropriate community services and equipment, and monitoring patient progress in collaboration with contracted service providers. When a patient's clinical or functional needs change, the Nurse Coordinator works with the appropriate healthcare professionals to reassess care requirements, facilitate timely escalation when indicated, and coordinate transitions to alternative services, including OH@home, where ongoing support is required.
As an integral member of Community Programs, the Nurse Coordinator supports patient flow, reduces avoidable hospital days and Alternate Level of Care (ALC) pressures, and promotes safe, effective care in the home. The position contributes to quality improvement, program evaluation, and the continued expansion of Southlake@home, supporting Southlake Health's commitment to integrated, population health-focused care and strengthening the connection between hospital and community services
