Human Touch Home Health is seeking a compassionate and motivated Physical Therapist (PT) to join our growing team serving patients throughout the I-95 Corridor from Triangle to Springfield, Virginia, including Triangle, Dumfries, Quantico, Woodbridge, Lake Ridge, Dale City, Lorton, Newington, and Springfield.

As a Home Health Physical Therapist, you'll provide one-on-one care that helps patients regain strength, improve mobility, restore balance, and return to the life they love.

What You'll Do
- Complete physical therapy evaluations, reassessments, and discharge visits
- Develop individualized plans of care based on patient goals and clinical needs
- Provide skilled physical therapy services in patients' homes
- Educate patients and caregivers on therapeutic exercises, mobility training, balance improvement, fall prevention, and home safety
- Monitor patient progress and modify treatment plans as appropriate
- Collaborate with physicians and the interdisciplinary care team
- Complete timely and accurate clinical documentation using WellSky
Qualifications
- Active Virginia Physical Therapist License Required
- Valid Driver's License and Reliable Transportation
- Strong Clinical Assessment and Documentation Skills
- Excellent Communication and Interpersonal Skills
- Home Health Experience Preferred but Not Required
- OASIS Experience Preferred
- Experience with WellSky Home Health is a plus
- Passion for Delivering Exceptional Patient-Centered Care
