Perioperative Assessment Clinic APP, Community Health Partners

Overview

Community Health Partners is seeking a full‑time Advanced Practice Provider (APPs) to join the Perioperative Assessment Clinic team. In this role, the APP will provide comprehensive perioperative care for elective surgeries and endoscopic procedures by collaborating with an interdisciplinary care team. Responsibilities include conducting detailed clinical assessments, developing and managing treatment plans, performing medical diagnoses and prescribing appropriate therapies and treatment plans. The APP will support patients throughout the surgical continuum, including pre-operative assessment and optimization as well as perioperative and postoperative care recommendations, ensuring the highest standards of safety and quality.

Responsibilities

 

In this role, you will:

• Works with the Clinic Medical Director and multidisciplinary team to develop, implement, evaluate, and refine evidence-based systems of care for preoperative evaluation and optimization.
• Serves as a primary clinical provider for patients moving through the preoperative assessment and optimization process, with physician consultation and support available as clinically indicated.
• Performs and documents comprehensive preoperative medical evaluations, including medical and surgical history, prior anesthesia or surgical complications, medication and allergy review, functional capacity, social factors, physical assessment, and standardized risk assessments.
• Communicates with patients and families by telephone, in person, through the electronic health record, and through other approved communication methods to obtain information, explain recommendations, and support shared decision-making.
• Completes and validates medication reconciliation; identifies high-risk medications, potential interactions, and perioperative medication-management needs; and prescribes, adjusts, or recommends therapy within scope of practice and approved protocols.
• Uses current evidence, standardized screening tools, and computerized clinical decision support to identify perioperative risk, determine the need for additional evaluation, and consistently apply best practices.
• Orders, reviews, and interprets laboratory studies, diagnostic tests, electrocardiograms, medical records, and specialty consultations, and takes appropriate action on abnormal or clinically significant findings.
• Diagnoses and manages acute and chronic conditions within scope of practice and develops individualized plans to optimize medical status, reduce perioperative risk, and prepare patients for surgery.
• Assesses medical readiness for surgery within clinical privileges and communicates recommendations to the patient, surgeon, anesthesia team, and other members of the care team, including recommendations to proceed, obtain additional evaluation, optimize further, or postpone when appropriate. Final anesthetic assessment remains with the responsible anesthesia professional.
• Coordinates care and closes communication gaps among patients, surgeons, anesthesia professionals, primary care clinicians, specialists, pharmacists, and hospital departments.
• Recognizes clinical instability, unresolved symptoms, or barriers to safe surgery and initiates timely escalation, urgent evaluation, or emergency response when indicated.
• Creates and communicates clear, patient-specific care plans, orders, and perioperative instructions and ensures that the preoperative record is accurate, complete, and available to the surgical and anesthesia teams.
• Provides clinical guidance to registered nurses and other clinic team members, supports consistent application of protocols, and promotes effective interdisciplinary teamwork without assuming formal supervisory responsibility unless assigned.
• Participates in clinical quality review, performance-improvement activities, protocol development, and analysis of clinic outcomes, and recommends changes that improve safety, reliability, access, and the patient experience.
• Maintains current knowledge of perioperative medicine through guideline review, continuing education, professional conferences, and active exchange of knowledge with colleagues.
• Provides direct assessment, evaluation and primary patient care, complying with medical protocols and procedures.
• Provides direct patient management in collaboration with physicians; participates in daily rounds, patient assessments and care plan development.
• Orders, interprets and evaluates diagnostic tests to identify and assess patient's healthcare needs. Furnishes medications based upon written protocols.
• Ensures that patients are counseled regarding treatment plans and the use, dosage and possible side effects of medications.
• Makes referrals to appropriate providers when patients require subspecialty medical attention beyond the area's scope of service.
• Documents and maintain precise and complete records of patients.
• Distinguishes between normal and abnormal findings.
• Provides education, training, information and consultation services to interdisciplinary team, patients, family and caregivers.
• Performs other duties as assigned.

Qualifications

Education and Experience Minimum Requirements:

• Completion of a Nurse Practitioner or Physician Assistant program required
• Credentialed by the CMC Medical Staff Office as an Allied Health Professional

Licenses and Certifications:

If qualifying as a Nurse Practitioner, the following is required

• NP - Current State of California Nurse Practitioner License required
• NPF - Nurse Practitioner Furnishing Number required

Experience

• Three of more years of experiences preferred in a perioperative area, time as a registered nurse counts

 Licenses and Certifications• Eligible to be licensed to practice Medicine in the state of California required
