Description
Overview
The Resource RN performs the patient/family interview, history, assessment, discharge planning and documentation process. The RN assessments include: skin/wound, fall risk, pain, home medications, allergies, psycho/social, spiritual/cultural, nutritional and functional. The assessment alos includes presence of central lines, pacemakers, or other assistive devices and a complete review of systems. The RN orients patient/family to room and unit, reviews admission packet (advanced directives, patients rights/complaints, pain, Speak Up initiative) and completes admission forms. The RN assists in explaining the Communication Board to the patient and family. The RN initiates telemetry monitoring. The RN identifies patients needs and priorities for care through assessment. The RN will initiate the Medication Reconciliation process.
The RN performs discharge teaching, documents, and discharges patients prior ro admissions arriving on unit and as available throughout shift
The RN assists the Unit Director in maintaining compliancy with hospital Core Measures and Quality compliance.
Qualifications
Education:
High School Diploma/GED required.
Graduate from and accredited School of Nursing required.
License, Certification and/or Registration:
Current Alabama or Multistate Nursing license required.
BLS for Healthcare Providers required by end of Orientation period.
Experience:
One year of medical/surgical nursing with admissions experience required.
Additional Skills/Abilities:
Assessment skills and good oral and written communication skills.
Proficient computer skills.
Follows HH guidelines for customer service and communication.
Observes key principles in patient, family and staff interactions
Utilizes critical thinking in performing a complete assessment on all patients based on unit scope of care.
- Takes appropriate action based on assessment findings
Initiates medication reconciliation per hospital guidelines.
Utilizes critical thinking skills in performing the discharge planning, teaching, documenting and discharge of patient.
Develops and implements interdisciplinary plan of care based on identified needs involving patient/family
Initiates interdisciplinary teaching based on identified needs involving patient/family
Documentation meets current HH standards and policies.
Assumes accountability for completing patient assessments, collecting, compiling and communicating data, which reflect the admission process.
Maintains patient confidentiality
Initiates telemetry monitoring
Maintains current knowledge with unit quality and assist in teaching information to unit staff.
Performs other duties as assigned.
