Location: Linden, MI
Concept Rehab is looking for a Physical Therapist (PT) to join our team on a PRN basis in Linden, Michigan.

What You’ll Do
- Evaluate patients and develop individualized treatment plans
- Provide skilled physical therapy services to help patients improve mobility, strength, balance, and independence
- Document patient progress and treatment according to professional standards
- Collaborate with nursing, therapy, and other members of the care team
- Provide compassionate, patient-centered care
What We’re Looking For
- Current Physical Therapist (PT) license in the state of Michigan
- Graduate of an accredited Physical Therapy program
- Strong communication and organizational skills
- Reliable and compassionate approach to patient care
- Previous experience in a skilled nursing or post-acute setting is helpful, but not required
