Description
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one physical therapy treatments in patients' homes.
- Address gait training, balance, strengthening, transfers, endurance, pain management, and fall prevention.
- Recommend assistive devices, durable medical equipment, and home safety modifications.
- Complete accurate and timely clinical documentation.
- Collaborate with physicians, caregivers, family members, and the interdisciplinary care team.
