Nurse Practitioner – Primary Care & Mental Health

HealthOPM is recruiting a Nurse Practitioner for a community-focused primary care role in Oshawa. You'll provide autonomous care to individuals and families across the lifespan, addressing the intersection of physical health, chronic disease, mental health and the social factors that influence wellbeing.

Your work extends beyond the clinic when needed, giving you the opportunity to build continuity with patients and connect them to the care, resources and supports that can make a meaningful difference.

Your Practice

Assess, diagnose, prescribe and manage care independently within your NP scope, while having physicians, specialists and allied health professionals around you for collaboration.

• Manage scheduled and urgent visits, chronic and complex health concerns, diagnostics, medications, treatment and follow-up.

• Provide counselling, health education, disease prevention and practical strategies that help patients take a more active role in their health.

• Deliver care in the clinic and community, coordinating with healthcare providers and community services when patients need support beyond a traditional appointment.

This isn't a narrowly defined NP position. You'll be expected to think critically, make clinical decisions and bring together the physical, psychological, social and environmental pieces of a patient's health. That breadth is directly reflected in the underlying role.

What You'll Bring

You are a confident, thoughtful clinician who can work independently without practising in isolation.

• Current CNO registration as an RN(EC)/Nurse Practitioner and recognized Primary Health Care NP certificate.

• Master's degree in Nursing or a health-related field, plus a BScN.

• Strong assessment, diagnostic, prescribing, care-planning and communication skills.

• Ontario driver's licence and required professional liability coverage.

Experience with EMRs and knowledge of psychotropic medications and Ontario mental-health legislation are assets. French is also considered an asset. These requirements and assets reflect the client's specified profile.
