Salary: $125000 – $147,500 per year
Schedule: Primarily Days | One Evening Shift Every Two Weeks | Flexible Hours

What You'll Do
As a Community Outreach Nurse Practitioner, you will:

• Host and provide care through community-based medical clinics
• Deliver primary healthcare services to individuals across the community
• Conduct home visits when required
• Lead vaccination and immunization clinics
• Collaborate with interdisciplinary teams and community partners
• Coordinate care and liaise with primary care and healthcare resources
• Maintain strict privacy, confidentiality, and professional standards
• Provide compassionate, trauma-informed care with empathy and respect
• Support individuals experiencing barriers to healthcare access

What We're Looking For
The ideal candidate will have:

• Completion of a recognized Primary Care Nurse Practitioner Program
• Bachelor of Science in Nursing (BScN)
• Current registration as a Nurse Practitioner (Extended Class) with the College of Nurses of Ontario
• A valid Ontario driver's licence, reliable vehicle, and automobile insurance
• Experience in primary care and/or community health is considered an asset
• Understanding of the social determinants of health and the impact of social isolation on individual and community well-being
• Excellent interpersonal, communication, and conflict resolution skills
• Strong emotional intelligence and the ability to establish healthy professional boundaries
• Excellent verbal and written English communication skills; additional languages are an asset
• Proficiency with Microsoft Office, email, and other computer applications
• Ability to obtain a satisfactory Police Vulnerable Sector Check
