Regular Schedule: 34 hours per week
 
 A Non-Merit vacancy exists in Community Health Services, Department of Health and Human Services. 
 
 Non-Merit positions are not classified within the Baltimore County Government Classification and Compensation Plan.
 
 A list of eligible applicants will be established based on the examination as outlined below.
 
 Current and future vacancies occurring in any Office or Department of Baltimore County General Government may be filled from the list of eligible applicants.

Under general supervision, performs routine community outreach duties for public health Chronic Disease Prevention (provides education, and assists with case management, outreach activities, in-home visits, community events and Risk assessments for Prediabetes, T-2 Diabetes, Hypertension ,cardiac , telephonic outreach, nutritional education and other duties as assigned. 

EXAMPLES OF ESSENTIAL DUTIES

  • Conducts telephonic, in-home, and community-based needs assessments and makes appropriate referrals based on clients’ needs related to chronic disease prevention for Prediabetes, hypertension and cardiac patients, linkages to resources, and health education, lifestyle changes, weight management.

  • Travels extensively to client homes, community locations, various agencies, and other outreach destinations.

  • Provides basic education and information about chronic diseases in a culturally competent manner.

  • Links clients to resources and distributes supplies to help facilitate adherence to shelter-in-place and disease containment.

  • Reinforces chronic disease management tools to reduce the impact of the disease to include, medication management tools, self -management of blood pressure, glucometer education, smoking cessation referrals.

  • Documents client encounters and contacts made on the behalf of clients; maintains electronic spreadsheet of client encounters; completes and submits all reports within the required timeframes.

  • Performs miscellaneous job-related duties as assigned

  • Works collaboratively and effectively within the Bureau of community health team, provides feedback to other members of the team.  

  • Builds and maintains cooperative and liaison relationships with public and private agencies.

Required Qualifications
Possession of a high school diploma or appropriate equivalent
plus 
 at least one year experience working with the public.
  
Knowledge, Skills, and Abilities
Knowledge of English grammar.  Knowledge of and sensitivity to cultural influences.

Ability to learn and follow county and departmental rules, regulations, policies, and procedures; ability to establish effective working relationships with clients, families, and public and private agencies; ability to communicate effectively with individuals and groups; ability to work flexible hours; ability to work in a variety of environments; ability to maintain documentation regarding activities; ability to maintain records, files, and reports; ability to coordinate activities; ability to maintain inventory of supplies.
 
Licenses or Certificates
Possession of a valid driver's license equivalent to a noncommercial, class C Maryland driver's license is required at the time of appointment.
  
Proof of Licenses, Certifications and Education
Applicants are required to submit proof of licenses, certifications and education beyond high school to meet the required and preferred qualifications of the position. Diplomas or transcripts must show the applicant's major field of study. Copies and unofficial transcripts are acceptable.

Proof of Degree Equivalency

EXAMINATION PROCEDURE
Applicants will be qualified based on an evaluation of their training and experience, as stated on their application, which includes answers to the supplemental questions.  Applicants must state the dates and duties of past and present experience clearly and completely for evaluation purposes.Persons appointed to this classification may be required to work evenings and a rare weekend.
  
The work of this classification requires traveling throughout the county, and therefore, requires daily availability of a vehicle. 
  
Training for this position will be provided; successful completion of the training is required.
  
Medical Examination and Employment Background Investigation
Applicants selected for an appointment to a position in Baltimore County must successfully complete a physical examination and drug screen and an employment background investigation, including, but not limited to a criminal background, education, and fingerprint check.
