Medical Coder (Hospital & Outpatient Coding)

Position Summary

We are seeking an experienced Medical Coder with a strong background in hospital and outpatient coding.

The ideal candidate will be responsible for reviewing medical records, consultation notes, and clinical

documentation to accurately assign ICD-10, CPT, and HCPCS codes. Experience with neurology is not

required; however, candidates must have hands-on experience coding hospital-based encounters and

procedures, including time-based services such as EEG studies.

Key Responsibilities

Review and analyze patient charts, consultation notes, operative reports, and clinical

documentation.

Assign accurate ICD-10-CM, CPT, and HCPCS codes for hospital and outpatient services.

Code diagnostic procedures and time-based services, including EEGs and other specialty

procedures, based on physician documentation.

Ensure coding accuracy, compliance, and adherence to payer and regulatory guidelines.

Identify and resolve documentation discrepancies by coordinating with providers and clinical

staff.

Maintain productivity and quality standards while meeting established turnaround times.

Stay current with coding updates, payer policies, and industry regulations.

Qualifications

Certified Professional Coder (CPC), CCS, or equivalent certification preferred.

Minimum 2 years of medical coding experience.

Required experience in hospital and/or outpatient coding.

Strong knowledge of ICD-10-CM, CPT, and HCPCS coding systems.

Experience reviewing medical charts, consultation notes, and clinical documentation.

Familiarity with time-based procedure coding, including EEG services.

Excellent analytical skills and attention to detail.

Proficient in EMR/EHR systems and coding software.
