Care Manager HourlyPosition Summary:
The Wellness Navigator is responsible for providing support to clients in Cape May County that need mental health services. Work to support clients to obtain treatment services and maintain care to achieve wellness. Work with a team to ensure clients needing help to achieve mental wellness are wrapped with all needs of service. Educate clients regarding their health care condition and provide follow-up with clients and providers to ensure client needs are met. Operate as a strong community liaison to ensure that individuals can navigate and understand all services that are available to them for their mental health needs. Collaborate with Acenda and Community Providers to link individuals into services needed.

Key Responsibilities:
- Serve as the contact point, advocate and informational resource for clients, care team, family/caregivers and community resources regarding client's treatment plan
- Partner with clients to monitor adherence to their treatment plan, evaluate effectiveness, monitor progress in a timely manner and facilitate changes as required.
- Facilitate client linkages, referrals and access to appropriate treatment, community supports and medical or specialty providers according to the treatment plan
- Educate patient and family/caregiver(s) about relevant community resources and provide resources/connection to IFSS.
- Conduct clinical follow up when clients are receiving inpatient or emergency care, and ensure a seamless transition back into the treatment plan upon discharge.
- Cultivate and maintain relationships with external community providers and supports.
- Completes and submits accurate reports and chart documentation in a complete and timely manner according to program guidelines and established procedures.
- Ensures quality and accuracy of written and verbal communications.
- Compile data for and prepare a variety of reports as required. May include financial, dashboards or market research to assist in business development and programming
- Support the Quality/Compliance/Risk function as required.
- Performs other related duties as assigned.

Education, Work Experience and/or Skills Requirements:
- Bachelor’s Degree with 2 years’ experience in the human services, psychology or related field
- Resident of the geographical areas that are identified or relevant knowledge of counties served.
- Must have an possess a valid driver's license, use of an insured vehicle, and an acceptable driving record
- Must be sensitive, flexible and responsive to gender, race, ethnicity, socio-economic status, religion, age, sexual orientation or any other special needs as reflected in the ability to communicate with staff and/or clients.
- Professionalism and commitment to excellence in care
Physical Requirements:
- While performing the duties of this job, the employee is occasionally required to remain in a stationary position, often standing or sitting for prolonged periods.
- Repeated motions that may include the wrists, hands and/or fingers
- The employee must occasionally move about to accomplish tasks, adjust or move objects up to 20 pounds.
- Must be able to communicate with others to exchange information
- Ability to drive a vehicle
