Human Touch Home Health has an immediate need for Physical Therapists (PTs) throughout Upper Marlboro and Southern Prince George’s County.
We are hiring for Full-Time, Part-Time, and PRN opportunities and looking for Maryland-licensed PTs who want flexibility without losing the opportunity to build a consistent caseload.
Our patients need more than exercises.
They need someone who can help them safely move through the place that matters most: home.

What You’ll Do
As a Home Health Physical Therapist, you will:

Perform comprehensive PT evaluations
-
Develop and update individualized plans of care
-
Assess strength, balance, gait, endurance, mobility, and functional limitations
-
Provide therapeutic exercise and mobility training
-
Provide gait, transfer, and stair training
-
Address fall risks and home-safety concerns
-
Develop individualized home exercise programs
-
Recommend assistive devices and adaptive strategies as appropriate
-
Educate patients and caregivers
-
Monitor patient progress and adjust treatment plans
-
Complete reassessments and discharge planning
-
Collaborate with physicians, nurses, therapists, and other members of the care team
-
Complete accurate and timely documentation using WellSky
What We’re Looking For
-
Active Maryland Physical Therapist License Required
-
Graduate of an accredited Physical Therapy program
-
Reliable transportation
-
Strong assessment and clinical decision-making skills
-
Excellent communication and documentation abilities
-
Ability to work independently in patients’ homes
-
Home Health Experience Preferred, but Not Required
-
WellSky Experience is a Plus
