Physical Therapist Opportunity - Skilled Nursing & Rehabilitation
Concept Rehab is seeking a Part-Time Physical Therapist (PT) to join our team in a Skilled Nursing Facility (SNF) in Delaware, Ohio. This position provides the opportunity to work with adult and geriatric patients receiving post-acute rehabilitation, skilled nursing care, and long-term care services.
As a Physical Therapist, you will evaluate and treat patients recovering from illness, injury, surgery, hospitalization, and functional decline. You will collaborate with physicians, nursing staff, rehabilitation professionals, patients, and families to develop individualized treatment plans that improve mobility, strength, balance, safety, function, and quality of life.

Requirements
Essential Responsibilities
Patient Evaluation & Treatment
- Perform physical therapy evaluations within established timelines following physician referral.
- Develop individualized plans of care based on patient needs and goals.
- Provide evidence-based physical therapy treatment interventions.
- Monitor patient progress and modify treatment plans as clinically appropriate.
- Complete discharge planning and recommendations.
Clinical Collaboration
- Communicate effectively with physicians, nurses, rehabilitation staff, patients, and family members.
- Participate in interdisciplinary team meetings and care planning discussions.
- Educate patients and caregivers regarding treatment goals, safety, and home or maintenance programs.
- Recommend adaptive equipment and therapeutic interventions that support optimal outcomes.
Documentation & Compliance
- Maintain accurate, timely, and professional clinical documentation.
- Complete treatment notes, progress reports, discharge summaries, and billing documentation in accordance with company policies and regulatory standards.
- Maintain productivity expectations while delivering high-quality patient care.
Leadership & Education
- Supervise Physical Therapist Assistants (PTAs) as required by state regulations.
- Provide in-service education and training to facility staff.
- Support restorative nursing and functional maintenance programs.
- Serve as a clinical instructor for affiliated physical therapy educational programs when applicable.
Required Qualifications
- Bachelor's, Master's, or Doctorate Degree in Physical Therapy (BS, MS, MPT, or DPT)
- Current Ohio Physical Therapist license in good standing
- Strong clinical reasoning and patient care skills
- Excellent communication and documentation abilities
- Ability to work collaboratively within an interdisciplinary rehabilitation team
Preferred Qualifications
- Skilled Nursing Facility (SNF) experience
- Long-Term Care experience
- Geriatric rehabilitation experience
- Post-acute care experience
- Experience supervising Physical Therapist Assistants (PTAs)
