At The Village at Rockville, we are seeking a dedicated Licensed Practical Nurse (LPN) MDS Coordinator to join our clinical team. As the LPN MDS Coordinator, you will play a vital role in ensuring accurate resident assessments, care planning, and reimbursement processes while supporting the delivery of exceptional care. Your work will directly contribute to the health, well-being, and quality of life of those we serve.
What You'll Do
- Coordinate and complete the Minimum Data Set (MDS) assessment process in accordance with federal, state, and organizational regulations.
- Collaborate with interdisciplinary team members to gather assessment data and ensure comprehensive resident evaluations.
- Assist in the development and updating of individualized care plans based on resident needs and assessment findings.
- Monitor assessment schedules and ensure timely completion and submission of all required documentation.
- Review clinical documentation for accuracy, completeness, and regulatory compliance.
- Support quality measures and performance improvement initiatives.
- Serve as a clinical resource for MDS-related regulations, documentation requirements, and reimbursement processes.
- Work closely with nursing leadership to promote excellence in resident care and regulatory compliance.
What You'll Bring
Required Qualifications
- Current Licensed Practical Nurse (LPN) license in the State of Maryland.
- Minimum of one (1) year of nursing experience in long-term care, skilled nursing, rehabilitation, or a related healthcare setting.
- Strong knowledge of nursing documentation, resident assessment practices, and care planning.
- Excellent organizational, analytical, and communication skills.
- Ability to manage multiple priorities while meeting deadlines.
- Ability to read and speak English.
Preferred Qualifications
- Previous MDS Coordinator or MDS experience.
- Experience with PointClickCare or other electronic health records systems.
- Knowledge of quality measures and regulatory compliance requirements.
Compensation & Benefits
Competitive Salary:
$70,000 - $90,000 annually (Final compensation will be based on experience, credentials, and demonstrated expertise.)
