Dignity Care Hospice Clinical Manager – Cincinnati, OH

Position Overview

The Clinical Manager at Dignity Care Hospice serves as both a clinical leader and operational steward, ensuring exceptional, compassionate, and compliant hospice care. This role integrates direct nursing practice with multidisciplinary team oversight, regulatory preparedness, quality improvement, and staff development. The Clinical Manager helps shape the patient and family experience while supporting the mission of providing dignified, meaningful end‑of‑life care.

Key Responsibilities

Clinical Leadership & Patient Care

• Complete initial and ongoing assessments of patients and families, including full physical evaluations and medical histories.

• Develop individualized plans of care based on terminal prognosis; revise plans as patient needs evolve.

• Administer medications and treatments safely and according to physician orders.

• Provide education, counseling, and health care instructions to patients and families.

• Ensure hospice care and services are delivered according to the plan of care and regulatory standards.

• Maintain skilled nursing competency and provide relief or direct patient coverage when needed.

• Participate in on‑call rotation.

Team Management & Staff Development

• Provide direction, coordination, and evaluation of hospice care delivered by the multidisciplinary team.

• Participate in hiring, orientation, retention, and competency verification of hospice staff.

• Conduct annual performance evaluations and ongoing supervisory visits with coaching and feedback.

• Hold staff accountable for compliance with regulations, policies, documentation standards, and care procedures.

• Recognize excellence and identify opportunities for improvement through clinical record review.

Quality, Compliance & Regulatory Preparedness

• Ensure care delivery aligns with hospice policies, Federal and State regulations, and accreditation standards.

• Prepare staff for successful regulatory and accreditation surveys.

• Maintain patient, family, and employee confidentiality.

• Participate in the hospice QAPI program, including data collection, analysis, and performance improvement initiatives.

• Monitor patient, family, and referral satisfaction; identify trends and address concerns.

Interdisciplinary Coordination

• Oversee care coordination among nursing, therapy, social work, spiritual care, physicians, and caregivers.

• Participate in case conferences, including instruction on therapy goals and interdisciplinary care strategies.

• Ensure staff appropriately inform patients and families regarding hospice services and associated costs.

Operational & Fiscal Responsibilities

• Support agency operations through reporting related to finance, billing, documentation, and outstanding patient items.

• Assist in developing and monitoring the branch budget; provide justification for variances.

• Promote sound business practices and support the fiscal success of the agency in alignment with organizational directives.

• Demonstrate the ability to assume Director responsibilities during temporary absences or vacancies.

• Contribute to team efforts and maintain a positive, accountable, and collaborative work culture.

Education, Licensure & Experience Requirements

• Graduate of an accredited school of nursing.

• Active RN license in the state of Ohio.

• One (1) to two (2) years of recent acute care experience required.

• Bachelor’s degree with one (1) year of hospice or home health experience preferred.

Schedule

This is a Full-time position, 8 am to 5 pm, Monday- Friday, with an on-call rotation.
