JOB SUMMARY:Within the context of a client and family-centred framework and in accordance with established standards of professional practice and the vision, values and policies of the organization, including Indigenous Cultural Safety, cultural humility and anti-racism principles, works in a team-based environment and is charged with providing discipline specific clinical support, training and education to designated staff for the designated program area a majority of the time. Plans, implements, and evaluates site educational programs to support access and flow and meet the needs of the clinicians and site. Provides training and at the elbow support; provides education on applicable legislation and its application, therapeutic approaches and discharge planning processes. As a Designated Responder Coordinator, provides clinical support, guidance, direction and education, and ensures Designated Authority of statutory obligations are met per Adult Guardianship Act. Works under the direction of the Manager and/or Team Lead to set the priorities of the designated program. Collaborates with the Regional Clinical Education team, Regional Educator for Social Work, and other educators across VCH to ensure alignment of best practices for educational theory and remain connected with developing best practices in the profession. Collaborates with the Professional Practice and Clinical Education team to ensure awareness of best practice evidence and newly emerging practices and engage with subject matter experts as appropriate. Maintains and updates own knowledge of clinical and professional practice, including clinical competencies by working directly with patients, clients, and residents. QUALIFICATIONS:Education & Experience Master’s degree in Social Work from a recognized program. Three (3) years recent, related experience working with clients with complex needs including in home health, primary care, mental health, and/or substance use, including one (1) years’ experience developing, implementing and/or delivering education services, or an equivalent combination of education, training and experience. Current registration with the British Columbia College of Social Workers.Valid BC Driver’s License for local area travel. Knowledge, Skills & AbilitiesKnowledge of the designated client population.Knowledge of substance use and a non-judgmental attitude towards working with clients with problematic substance use through their recovery. Comprehensive knowledge of the application of appropriate Acts (e.g. Adult Guardianship, Mental Health Act) and legislation as it relates to the rights and obligations of clients and staff. Demonstrated ability to conduct comprehensive risk assessments and create care plans to mitigate identified risksBroad knowledge of evidence-based practice, as it relates to mandates and policies of other government agencies including Home and Community Care, Ministry of Human Resources and Public Guardian and Trustee.Broad knowledge of adult education theory, and recognized experience with curriculum and instructional design principles and practices.Broad knowledge of therapeutic counselling theories, principles, and approaches.Knowledge of case management and social work practice within a client and family-centred care model for clients with complex health care needs.Comprehensive knowledge of other health disciplines and their role in client care.Broad knowledge of research methodology and processes.Comprehensive knowledge of Trauma Informed Practice, Cultural Safety, Harm Reduction and Recovery Oriented Care.Broad knowledge of behaviour management and ability to utilize non-violent crisis intervention strategies in a trauma informed manner.Demonstrated knowledge of the impact of colonization on Indigenous peoples.Broad knowledge of facilitation, mediation, and conflict resolution techniques and demonstrated analytical and problem-solving skills.In-depth knowledge of VCH clinical documentation systems including PARIS, Intra-Health Profile EMR.Demonstrated ability to teach, demonstrate, facilitate and lead/coach.Demonstrated ability to use critical thinking in approach to teaching.Demonstrated ability to develop educational programs and material.Communicates effectively and in a culturally safe manner with clients, caregivers and health care team, either one-on-one or in-groups.Demonstrated ability to function as an effective team member, leader, and facilitator.Demonstrated ability to provide leadership and work direction.Demonstrated ability to promote a creative learning environment.Demonstrated ability to establish workload priorities in collaboration with others.Demonstrated ability to provide effective consultation.Demonstrated ability to work independently and in collaboration with others.Physical ability to perform the duties and responsibilities of the position.The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.
