The Manager, Patient Care is responsible for the quality, safety and efficiency of patient care providers in the assigned program. This includes functional responsibility for the day-to-day operation of all components of the program and integration into the VNA and SLHHN. Assumes leadership role in planning for the program including but not limited to personnel, performance improvement, patient safety, regulatory compliance, programs/services, and physical plant issues as appropriate. Provides oversight of all patient care services and personnel to include: making patient care assignments, coordinating patient care, coordinating referrals, assuring that patient needs are continually assessed, assuring the development, implementation and updates of the individualized plan of care.

JOB DUTIES AND RESPONSIBILITIES:

  • Assumes 24-hour responsibility/accountability for planning, organizing and supervising care necessary to assure patient safety and meet the needs of patients served by assigned program.

  • Performs direct administrative functions for assigned program.

  • Develops, implements and evaluates short and long range performance improvement and operational goals and objectives for the department(s).

  • Implements policies, procedures, rules, regulations and recommended practices and ensures that clinical practice standards are met.

  • Analyzes and responds accordingly to program specific performance, outcome and financial data.

  • Develops capital and operational budgets in collaboration with Program Director and Finance.

  • Selects, motivates and retains staff; initiates formal recognition plan.

  • Develops, evaluates and adjusts current and future staffing based upon patient care needs and initiates adjustments as needed; manages costs within budget.

  • Maintains departmental records for administrative and regulatory purposes.

  • Facilitates unit based councils and participates in network and entity councils.

  • Enhances job knowledge by remaining current in trends in the health care industry and specialty areas.

  • Participates in selection and termination of staff.

  • Works closely with appropriate management and administrative staff in the development and attainment of program goals.

  • Serves as a resource to staff.

  • Leads and/or participates in projects involving performance improvement activities.

  • Promotes an environment that fosters inquiry, creative thought, nursing education programs, and development and application of evidence-based practice initiatives.

  • Reviews performance of staff, assures staff competency and initiates remedial actions, when appropriate.

  • Assists in the development and maintenance of staff competencies.

  • Organizes monthly committee/staff meetings as appropriate.

PHYSICAL AND SENSORY REQUIREMENTS:

Involves exerting 50-100 pounds of force occasionally, or 25 to 50 pounds of force frequently, or 10-to 20 pounds of force constantly to move objects. Constantly standing and walking. Physical demand requirements are in excess of those for Medium, Light and Sedentary work as defined by U.S. DOL. Reaching: Frequent. Reaching Above the shoulder: Frequent. Twisting: Frequent. Bending/stooping:  Frequent. Repetitive movements of elbows/fingers:  Frequent. Repetitive movements of wrists:  Frequent. Kneeling/Climbing /Squatting:  Occasionally. Good physical and mental health. Neat, clean, free from body odors. Conforms to uniform code Visual and auditory acuity are required to provide comprehensive care.

EDUCATION:

Registered Nurse License. Bachelor in Nursing or a related health care field required upon hire or at time of promotion. 

TRAINING AND EXPERIENCE:

Minimum of three (3) years clinical experience with demonstrated leadership abilities required.

Sound clinical knowledge in area to be managed. Strong interpersonal skills. Demonstrate ability in Human Resources and Financial Resource Management. Current BLS certification. Basic computer skills.
