The Social Worker II, as part of the Advanced Primary Care team, partners with patients to address social determinants of health needs across the continuum of care. This role provides ongoing support to patients and families by developing, implementing, and coordinating individualized care plans. The Social Worker II receives referrals for at-risk populations from interdisciplinary team members (including physicians, nurses, advanced practitioners, and other care team members) and proactively engages patients to improve health outcomes, enhance care coordination, and reduce barriers to care. Position Summary
The Social Worker - Population Health plays a key role in supporting patients across the continuum of care by addressing social determinants of health, connecting individuals with community resources, and collaborating with interdisciplinary care teams to improve health outcomes. This position works closely with patients, families, providers, and community partners to identify barriers to care and develop individualized plans that promote wellness, independence, and access to needed services.
This hybrid role offers a combination of remote work and on-site collaboration while supporting Mercy's mission of compassionate, patient-centered care.
Key Responsibilities
- Assess patients' psychosocial, environmental, financial, and support needs.
- Develop and implement individualized care plans that address barriers to health and wellness.
- Connect patients and families with community resources, financial assistance programs, behavioral health services, transportation resources, and other support systems.
- Collaborate with physicians, nurses, care managers, and other members of the interdisciplinary team to coordinate care and improve patient outcomes.
- Provide crisis intervention, advocacy, and ongoing support for patients with complex medical and social needs.
- Document assessments, interventions, and outcomes in accordance with organizational and regulatory requirements.
- Monitor patient progress and adjust care plans as needed to support successful health outcomes and reduce avoidable utilization.
- Promote patient engagement, self-management, and preventive care initiatives.
Qualifications
- Bachelor's degree in Social Work required; Master's degree in Social Work preferred.
- Current social work licensure, if required by state regulations.
- Previous experience in healthcare, case management, care coordination, population health, community health, or social services preferred.
- Strong knowledge of community resources and social service programs.
- Excellent communication, organizational, and relationship-building skills.
- Ability to manage multiple priorities in a fast-paced, collaborative environment.
