Hours Per Week: 40
Schedule: Monday - Friday 8a - 4:30p, 1 weekend per qtr, 1 winter and 1 summer holiday.

RESPONSIBILITIES
- Patient Care. Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care;
- Communication. Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problems
- Evaluation & Documentation. Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staff
REQUIRED QUALIFICATIONS:
- Nursing Diploma required
- License and current registration to practice as Registered Professional Nurse in New York State
- Current NYS Driver’s license
PREFERRED QUALIFICATIONS:
- Bachelors degree of Science in Nursing
- 1 year of experience working with a frail or elderly population preferred.
- Availability of Motor Vehicle
- 6 months- 1 year experience conducting UAS-NY Community Assessment preferred.
