DEFINITION
Under general supervision of the Children’s Behavioral Health Supervisor, the CCS/CLTS Case Manager provides the full range of Comprehensive Community Services (CCS) Service Facilitation and related Children’s Long-Term Support (CLTS) case management duties assigned to CCS/CLTS Case Managers.
EXAMPLES OF DUTIES
YOUTH CONNECT CRISIS STABILIZATION PROGRAM: Complete comprehensive assessment to inform family-driven and youth-guided crisis and crisis response plans; Write initial crisis and crisis response plans, in collaboration with PP-SSS, youth, and caregivers; Respond to youth and their caregivers to de-escalate behaviors, prevent return to inpatient setting, and establish safety plan; Assist family in recognizing high risk behaviors; teach, model, and practice coping skills and effective de-escalation interventions; Complete and document suicide risk assessment during interactions with youth; Assist family as described in A4 above, in-person, at a community based location, an average of once per week, at a time convenient for families; Collaborate with family to identify areas of need and provide direct assistance with securing resources to meet long-term needs; Engage and collaborate with systems actively involved with the family (e.g., Child Protective Services, Youth Justice, school, etc.), and identify and foster natural support systems; Establish support and service linkage plan, complete necessary referrals, and maintain communication with all programs referred to; Identify items needed to support safety and stabilization, and request use of wrap-around funds as needed; Maintain regular communication with providers to ensure continuity of services; Coordinate and convene regular team meetings. Maintained detailed, timely, and accurate clinical records, including service notes for all interactions; Obtain Releases of Information, and share information with system partners to promote continuity of care; Transport, or coordinate transportation, as needed to support processes of stabilization.
COMPREHENSIVE COMMUNITY SERVICES (CCS)/CHILDREN’S LONG-TERM SUPPORT (CLTS): Conduct comprehensive assessments of client support and service needs; Create person-centered Recovery Plans and CLTS support plans for eligible children and youth; Authorize appropriate therapeutic and supportive services; Monitor service utilization and effectiveness and update plans as indicated; Conduct recovery team meetings minimally every other month; Maintain regular contact with service providers and collaterals via phone, email and other correspondence; Update and adjust services as needed to ensure all of the client’s identified functional needs are met; Assist with crisis related client challenges as needed during office hours; Conduct home visits; Work with the Supervisor to report any critical incidents or use of restrictive measures to the Wisconsin Department of Health Services.
COMPREHENSIVE COMMUNITY SERVICES (CCS): Under the direction of a Social Work Supervisor, incumbents determine program eligibility for persons with mental illnesses and/or substance use disorders seeking Medicaid CCS. Meet with persons who have a mental health and/or substance use diagnosis to educate them about the features of the CCS program and its Recovery model. Ascertain voluntary interest in CCS participation. Gather assessment information via direct client interview and relevant collateral contacts. Complete Department of Health Services (DHS) functional screen to determine functional eligibility. Assist consumer with obtaining physician's prescription as necessary. Ensure that all required documentation is complete and filed in a timely manner. Assist consumers with selecting a service facilitation agency and facilitate a warm transfer to that agency for ongoing services. Assess need for immediate services and make arrangements if necessary. For those determined ineligible or not interested in CCS, provide information and assistance in accessing alternative resources. Perform annual reassessments of eligibility with DHS functional screen. Provide community education on the CCS program. Outreach to other service access points. Perform other duties as assigned.
COMMUNITY RECOVERY SERVICES (CRS): Under the direction of a Behavioral Health Program Specialist, incumbents provide direct service, program oversight, provider education, and community program coordination of the CRS benefit to eligible consumers with severe and persistent mental illnesses. Gather clinical assessment information via direct client interview and relevant collateral contacts in order to accurately complete WI DHS Mental Health/Alcohol and Other Drug Abuse (MH/AODA) Functional Screen to determine functional eligibility for CRS and need for psychosocial rehabilitation. Manage active CRS caseload, including: coordination with clients, case managers, and other community service providers; completing intake, 6-month updates, annual recertification and all required documentation and submitting to WI DHS; setting placement rates based on clinical need through use of the residential rate setting tool and individual placement agreements. Oversee Medical Assistance (MA) billing for services and identify, address, and resolve any issues, in collaboration with Dane County Department of Human Services (DCDHS) billing staff. Understand and implement all CRS benefit rules, updates, and changes from the Wisconsin DHS and ForwardHealth. Conduct provider oversight to ensure quality service provision and compliance with all CRS standards and requirements. Maintain accurate tracking of CRS revenues and reconciliation, documentation, reviews and re-certifications, provider compliance, and other quality assurance protocol established by DCDHS. Provide education, training, and technical assistance regarding the CRS benefit, including the CRS application and referral process, service delivery requirements, staffing requirements, and documentation requirements. Perform other duties as assigned.
Education, Training and Experience:
Requires a Bachelor’s or Master's Degree in a human services field (social work, psychology, sociology, nursing, counseling, etc.) and the equivalent of one (1) year of full-time experience working in a treatment setting with children and youth diagnosed with mental health disorders, substance use disorders, or developmental or intellectual disabilities. Experience is defined as working directly with individuals identified as part of the target population in a setting where the primary function is assessment, treatment, intervention, or observation of mental health and/or substance use conditions. Applicants who are currently in their final semester of school and are obtaining a Bachelor’s or Master’s Degree in a human services field will also be screened through.
Certifications/Licenses:
Candidates are required to pass relevant testing to become a certified Wisconsin Functional Screener within the first 90 days of employment.
Special Requirements:
Must have a valid driver’s license, a good driving record and access to reliable personal transportation, or equivalent. Availability to work evenings as required by family need. Also refer to the “Physical and Environmental Work Requirements” section below.

For Bilingual Positions:
- Provide services in both English and the required foreign language (e.g., Spanish, Arabic, Hmong, etc.).
- Applicants being considered for bilingual positions will be tested during the recruitment process prior to employment. This will primarily consist of passing an oral language proficiency assessment from English to foreign language and foreign language to English, but may require additional language testing, such as written translation, if it is an essential function of the job.

Physical and Environmental Work Requirements:
Requires sitting and standing; work inside and outside of an office environment which includes home visits and meetings with service providers. Ability to sit or stand to operate a personal computer with or without assistance. Ability to travel from office to other locations. Professional office work environment. Work involves walking, sitting, standing, bending, stooping, twisting, turning, lifting, carrying, pushing, pulling, grasping, and reaching within normal ranges. Ability to lift and move a variety of work-related materials weighing up to 25 pounds (with/without assistance). Prolonged computer and phone use. Manual dexterity using fine and gross motor skills. Ability to speak and be heard by others. Ability to see and hear within normal ranges with or without corrective devices.
